摘要
目的研究HLA-DRB1基因和终末期肾衰竭的相关性,寻找终末期肾衰竭患者的易感基因。方法采用聚合酶链反应序列特异性引物(PCR-SSP)分型方法对490例终末期肾功能衰竭患者进行HLA-DRB1分型,采用直接计数法计算基因频率,利用Haldane相对危险度修正公式计算该基因与疾病发生的相对危险度RR,通过乘以所比较的抗原总数对其进行修正。结果 490例终末期肾衰竭患者中共检出HLA-DRB1基因座等位基因13个,检出频率最高的等位基因为DRB1*15(0.167347),其次是DRB1*04(0.129592)、DRB1*12(0.126531)、DRB1*07(0.121429)、DRB1*09(0.088776),DRB1*11(0.088776),DRB1*14(0.082653),患者组DRB1*04、DRB1*12、DRB1*14基因与终末期肾衰竭的发生显著关联,DRB1*09对终末期肾衰竭的发生有保护作用。结论推测DRB1*04、DRB1*12、DRB1*14是河北汉族终末期肾衰竭的易感基因,而DRB1*09为保护基因。
Objective To study the correlation between HLA-DRB1 gene and end-stage renal disease in Hebei Han population,and to find the susceptibility genes of patients with end-stage renal disease. Methods HLA-DRB1 typing was performed on 490 patients with end-stage renal disease by polymerase chain reaction-specific primers(PCR-SSP).The frequency was calculated by direct counting method. Haldane's correction formula was used to calculate the relative risk of the HLA-DRB1 gene to the disease. The significance of RR was calculated by Fisher's exact P-value method and then corrected by multiplied the number of antigens. Results 13 HLA-DRB1 alleles were detected in 490 patients with end-stage renal disease. The highest allele was DRB1*15(0.167 347),followed by DRB1*04(0.129 592),DRB1*12(0.126531 DRB1*04(0.082 776),DRB1*11(0.088 776),DRB1*14(0.082 653),tested significance of RR found that the DRB1*04,DRB1*12,DRB1*14 gene significantly associated with end stage renal disease occurrence while the DRB1*09 have a protective effect on the of end stage renal disease. Conclusion DRB1*04,DRB1*12 and DRB1*14 are the susceptibility genes of end-stage renal disease in Hebei Han nationality,while DRB1*09 is a protective gene.
