罗哌卡因复合地佐辛对颅脑手术患者围术期氧化应激反应及血浆细胞因子的影响

摘要
目的:探讨罗哌卡因复合地佐辛对颅脑手术患者围术期氧化应激反应及血浆细胞因子的影响。方法:选择行颅脑手术的80例患者作为研究对象,按随机数字表法分为对照组和观察组,每组40例。对照组采用地佐辛进行麻醉,观察组给予罗哌卡因复合地佐辛麻醉。比较两组患者麻醉效果、不良反应发生率以及麻醉诱导前(T0)、术后1 h(T1)、术后6 h(T2)、术后24 h(T3)的血糖(Glu)、血浆皮质醇(Cor)、丙二醛(MDA)、一氧化氮(NO)、血浆肿瘤坏死因子(TNF-α)和可溶性细胞间黏附分子-1(sICAM-1)水平。结果:观察组镇痛效果持续时间显著长于对照组,气管拔管时间和麻醉恢复时间显著短于对照组(均P<0.05);麻醉后,与T0比较,两组T1、T2、T3时点时点血浆Glu、Cor、NO、MDA、TNF-α和sICAM-1水平升高,且同一时点观察组各指标均低于对照组,差异均具有统计学意义(均P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:罗哌卡因复合地佐辛对颅脑手术患者麻醉效果满意,可降低围术期氧化应激和炎症反应,具有较好的颅脑保护作用,且安全性好。
        Objective:To investigate the effect of ropivacaine and dezocine on perioperative oxidative stress and plasma cytokines in patients undergoing cerebral surgery.Methods:A total of 80 patients who underwent cerebral surgery were randomly divided into a control group and an observation group,with 40 cases in each group.The patients in the control group were anesthetized with dezocine,and those in the observation group were anesthetized with ropivacaine and dezocine.The anesthetic effects and the incidence of adverse reactions as well as the levels of blood glucose(Glu),plasma cortisol(Cor),malondialdehyde(MDA),nitric oxide(NO),tumor necrosis factor(TNF)-α and soluble intercellular adhesion molecule(sICAM)-1 before the induction of anesthesia(T0),and 1 h(T1),6 h(T2),24 h(T3) after surgery were compared.Results:The duration of analgesia in the observation group was significantly higher than that in the control group,while the extubation time and anesthesia recovery time were significantly lower(P<0.05).After treatment,the levels of Glu,Cor,NO,MDA,TNF-α and sICAM-1 in the observation group were significantly higher than those in the control group at the same time point(P<0.05).No significant difference in the incidence of adverse reactions was noted between the two groups(P>0.05).Conclusion:The anesthetic effect of ropivacaine combined with dezocine for cerebral surgery was satisfactory.This combination could reduce perioperative oxidative stress and inflammatory reaction,and was safe.
