血清β-羟丁酸检测在糖尿病酮症酸中毒临床诊疗中应用分析

摘要
目的研究血清β-羟丁酸水平在糖尿病酮症酸中毒的诊断和治疗中的临床意义。方法随机选择46例糖尿病酮症酸中毒患者作为酮症组,46例糖尿病无酮症患者作为非酮症组,均于2015年3月—2016年2月间入院治疗,另选同期体检的46名健康受检者作为对照组。所有患者均于入院次日检测血清β-羟丁酸、空腹血糖以及常规检查项,统计阳性患者例数和阳性率;治疗1周后,比较治疗前后和组间内酮症组和非酮症组患者的血清β-羟丁酸水平。另外,分析β-羟丁酸与空腹血糖的相关性。结果 (1)酮症组患者血清β-羟丁酸与空腹血糖水平显著高于非酮症组和对照组,尿酮体阳性率为100.0%,显著高于非酮症组和对照组,差异有统计学意义(P<0.05)。(2)治疗后,酮症组和非酮症组患者的血清β-羟丁酸水平均有显著降低,差异有统计学意义(P<0.05);组间比较,治疗前和治疗后,酮症组患者的血清β-羟丁酸均显著高于非酮症组患者,差异有统计学意义(P<0.05)。(3)经分析,酮症组患者的β-羟丁酸水平与空腹血糖含量呈显著正相关性(r=0.674,P<0.05)。结论血清β-羟丁酸检测在糖尿病酮症酸中毒的诊断和治疗中具有重要意义,对临床疾病诊治和病情判断具有重要参考价值。
        Objective To research the significance of serum β-hydroxybutyric test in clinical diagnosis and treatment of diabetic ketoacidosis. Methods 46 cases of patients with diabetic ketoacidosis were randomly selected as the ketosis group,46 cases of diabetes patients without ketosis were selected as the non-ketosis group, and all patients were treated from March 2015 to February 2016, and 46 cases of physical examination people at the same period were selected as the control group. Results The serum β-hydroxybutyric and fasting glucose in the ketosis group were higher than that of the non-ketosis group, the ketoacidosis positive rate was 100%, significantly higher than that of the non-ketosis group, the difference was statistically significant(P<0.05). The serum β-hydroxybutyric level of both groups after treatment decreased obviously(P<0.05), it was statistically significant. The serum β-hydroxybutyric of ketosis group was higher than that of the non-ketosis group(P<0.05), the difference was statistically significant. The serum β-hydroxybutyric and fasting glucose have positive correlation between each other(r=0.674, P<0.05) in the ketosis group. Conclusion The serum β-hydroxybutyric test is of important significance to the clinical diagnosis and treatment of diabetic ketoacidosis and is of important reference value to the clinical diagnosis and treatment and determination of diseases.
