社区健康管理在老年人慢性病控制中的效果分析

摘要
目的研究社区健康管理在老年人慢性病控制中的效果。方法选取2017年6月至2018年1月本院接收的某社区自愿参与社区健康管理的150例老年人为观察组,同时选取150例未参与社区健康管理的老年人为对照组,评估社区健康管理的应用效果。结果比较两组观察对象对慢性病知识的知晓率情况,观察组明显优于对照组(P <0.05);比较两组观察对象行为方式情况,观察组明显优于对照组(P <0.05)。结论社区健康管理在老年人慢性病控制中发挥着重要作用,有助于加深老年人对疾病的认识,逐渐改善患者的行为方式。
Objective To study the effect of community health management in the control of chronic diseases in the elderly. Methods One hundred and fifty elderly people who volunteered to participate in community health management from June 2017 to January 2018 were selected as the observation group,and 150 elderly people who did not participate in community health management were selected as the control group to evaluate the application effect of community health management. Results The awareness rate of chronic disease knowledge in the observation group was significantly better than the control group(P<0.05). The behavior of the observation group was significantly better than the control group(P<0.05).Conclusion Community health management plays an important role in the control of chronic diseases in the elderly,which helps to deepen their understanding of diseases and gradually improve the behavior of patients.
