摘要
扩张型心肌病是一类以左心室或双心室扩大伴收缩功能障碍为特征的心肌病,本病患病率约为1/2500,预后差,是心力衰竭的第三位原因,确诊后5 a生存约50%,10 a生存约25%。心脏再同步化治疗(CRT)植入同步化治疗除颤器(CRTD)等治疗手段与单纯药物治疗可有效预防心脏性猝死、降低死亡率、改善心功能,但价格昂贵、不能根本改善预后,基因治疗尚处于研究阶段,难度较大。心脏移植是最有效的根治方法,但因供体短缺、排异反应等无法广泛开展。自体骨髓单核细胞移植治疗可实现心肌细胞再生、改善新功能,进而提高生存率。同时,miRNA与扩张型心肌病的发生转归有相应的影响,本文就miRNA在自体骨髓单核细胞移植治疗扩张型心肌病中意义进行综述。
        Dilated cardiomyopathy is a kind of cardiomyopathy characterized by left ventricular or double ventricular enlargement and systolic dysfunction.The prognosis is poor and the prevalence rate is about 1/2500.It is the third cause of heart failure.The 5 year survival is about 50%,and the 10 year survival is about 25%.Cardiac resynchronization therapy(CRT) implantation of synchronous treatment defibrillator(CRTD) and other treatment methods and simple drug treatment can effectively prevent sudden cardiac death,reduce mortality,improve cardiac function,but the price is expensive,can not improve the prognosis,gene therapy is still in the study stage,it is difficult.Heart transplantation is the most effective radical treatment,but donor shortage and rejection can not be widely carried out.Autologous bone marrow mononuclear cells transplantation can achieve cardiomyocyte regeneration,improve new functions,and improve survival rate.At the same time,miRNA has a corresponding effect on the outcome of dilated cardiomyopathy.This article reviews the significance of miRNA in the treatment of dilated cardiomyopathy with autologous bone marrow mononuclear cell transplantation.
