补肺活血胶囊辅助治疗耐多药结核的近期疗效观察

摘要
目的:观察补肺活血胶囊辅助治疗耐多药结核的近期疗效。方法:将耐多药肺结核病例随机分为治疗组和对照组(n=21)。两组均采用6ZKmLfxPtoCS (PAS)/18ZLfxPtoCS (PAS)进行抗结核治疗,治疗组在此基础上口服补肺活血胶囊,疗程6个月。观察治疗前后痰结核菌转阴、痰结核培养转阴、胸片X线病灶吸收、空洞闭合、T细胞亚群检测(CD4,CD8)和肺功能变化情况,并监测药物不良反应。结果:治疗6个月末,两组患者的痰结核菌转阴率、痰菌快速培养阴转率无统计学差异。治疗组病灶吸收率、空洞闭合率、第1秒用力呼气容积(FEV1)、FEV1预计值、第1秒用力呼气量占用力肺活量比值(FEV1/FVC)及血清CD4~+,CD8~+和CD4~+/CD8~+较对照组有统计学差异(P <0. 05)。未发现补肺活血胶囊相关药物不良反应。结论:补肺活血胶囊辅助治疗耐多药肺结核可有效改善肺功能,促进病灶吸收好转,促进空洞愈合,改善患者的细胞免疫功能,且安全性高。
Objective: To evaluate the short-term efficacy of Bufei Huoxue Capsule in the treatment of multi-drug resistant pulmonary tuberculosis. Methods: A total of 42 cases diagnosed with multi-drug resistant pulmonary tuberculosis were randomly divided into treatment group and control group( n = 21). The two groups were both treated with chemotherapy 6 ZKmLfxPtoCS( PAS)/18 Z Lfx Pto CS( PAS),while Bufei Huoxue Capsule was used in the treatment group,and chemotherapy was only used in control group. The course of treatment is 6 months. Rates of sputum smear and culture negative conversion of bacteria,efficacy of X-rays of both groups after6 months treatment,T cell subgroup detection( CD4,CD8),lung function,and the adverse reaction were recorded. Results: The difference of negative conversion rate of bacteria( including sputum smear and sputum rapid culture) between both groups after 6 months of treatment has no statistical significance( P > 0. 05). The rate of focus absorption and cavity closure,the FEV1,the expectation of FEV1,FEV1/FVC and CD4~+,CD8~+,CD4~+/CD8~+of serum of the treatment group was significantly different from those of the control group( P < 0. 05).No adverse reactions related to Bufei Huoxue Capsule were found. Conclusion: The combination of Bufei Huoxue Capsule for multi-drug resistant pulmonary tuberculosis can effectively and safely improve the lung function,and also can improve the rate of focus absorption,promote the healing of cavity,and improve the cellular immune response of the patients with high safety.
