摘要
目的探讨新疆地区实习护生在临床实践过程中发生针刺危险的特点,并提出针对性建议。方法 2016年7月至2018年7月,便利抽样法选取在新疆乌鲁木齐市某三级甲等医院的实习护生140名为研究对象。按实习期间发生针刺伤或险些发生针刺伤的次数的不同将其分为高危组和低危组,使用统计学方法探讨发生针刺危险的相关因素。结果两组实习护士在学历构成上的差异有统计学意义(P<0.05),高危组大专学历构成比例高于低危组。两组实习护生在民族构成、年龄和实习时间上的差异无统计学意义(均P>0.05)。护生实习困难与针刺高危呈正相关(r=0.611,P=0.021);护生沟通能力与针刺高危呈负相关(r=-0.340,P=0.011)。针刺伤高危组累计实习时间较低危组短,差异有统计学意义(Log-rankχ~2=39.035,P<0.001)。高危组护生考核成绩累计得分低于低危组,差异有统计学意义(Log-rankχ~2=4.289,P<0.05)。实习时间、是否实习困难及操作考核成绩是影响实习护生针刺危险发生的独立因素。结论实习护生发生针刺伤危险与实习时间、实习困难及操作考核成绩较差有关,临床带教过程中可以根据上述结果采取针对性的干预措施。
Objective : To explore the characteristics of needling hazards among intern nurses in Xinjiang during clinical practice,in order to put forward pertinent suggestions.Methods From July 2016 to July 2018,140 intern nurses from a tertiary hospital in Urumqi,Xinjiang,were selected by convenience sampling method. The incidence of needle injury was counted and the related factors of needle injury were analyzed.Results There was significant difference in academic degree between the two groups(P< 0.05). The proportion of junior college education in high-risk group was higher than that in low-risk group. There was no significant difference in ethnic composition, age and the duration of internship between the two groups(P> 0.05). Nursing students' difficulty in practice was positively correlated with the high risk of needle injury(r=0.611, P=0.021), while nursing students' communication ability was negatively correlated with the high risk of needle injury(r=-0.340, P=0.011). The cumulative practice time of high-risk group was shorter than that of low-risk group(Log-rank χ~2=39.035,P<0.001). The cumulative scores of nursing students in high-risk group were lower than those in low-risk group(Log-rank χ~2=4.289,P<0.05). The internship time, difficulty in practice and performance of operation assessment were independent factors influencing the occurrence of needle injury among Intern nurses.Conclusions The risk of needle stick injuries among clinical nursing students were related to time of practice, the difficulties of practice operation and poor examination results. In the course of clinical teaching, targeted interventions can be taken according to the above results.
