益肺消积方对晚期非小细胞肺癌患者血清肿瘤标志物及生活质量的影响

摘要
目的:观察益肺消积方对晚期非小细胞肺癌患者血清肿瘤标志物及生活质量的影响。方法:80例晚期非小细胞肺癌患者随机分为对照组和观察组各40例。对照组给予GP方案;观察组在对照组治疗基础上给予自拟益肺消积方治疗,药物组成:黄芪30 g,麦冬15 g,沙参15 g,太子参20 g,丹参20 g,当归15 g,百合15 g,梨皮20 g,三七粉(冲)6 g,蜈蚣粉(冲)3 g,全蝎粉(冲)3 g,莪术15 g,白花蛇舌草12 g,半枝莲10 g,甘草9 g。比较两组患者治疗前后证候积分、血清肿瘤标志物水平及生活质量。结果:治疗后两组患者中医证候积分与治疗前比较,差异均有统计学意义(P<0.05);治疗后观察组患者中医证候积分显著低于对照组(P<0.05)。治疗后两组患者血清血清肿瘤标志物糖类抗原125(carbohydrate antigen 125,CA125)、癌胚抗原(carcino-embryonic antigen,CEA)水平与治疗前比较,差异均有统计学意义(P<0.05);治疗后观察组患者血清CA125、CEA及细胞角蛋白19片段(cytokeratin-19 fragment,Cyfra21-1)水平显著低于对照组(P<0.05);治疗后观察组患者血清Cyfra21-1水平显著低于治疗前(P<0.05)。观察组患者生活质量稳定率为77.5%,对照组为55.0%,两组患者稳定率比较,差异有统计学意义(P<0.05)。结论:益肺消积方治疗晚期非小细胞肺癌,可改善症状,降低患者血清肿瘤标志物水平,稳定生活质量。
Objective: To explore the effect of Yifei Xiaoji Prescription on serum tumor markers and quality of life in patients with advanced non-small cell lung cancer. Methods: 80 cases of advanced non-small cell lung cancer patients were randomly divided into control group and observation group,with 40 cases in each group. The control group was treated with GP scheme,and the study group was treated with Yifei Xiaoji Prescription in addition to that of the control group. Ingredients: Astragalus 30 g,Ophiopogon japonicus 15 g,Radix ginseng 20 g,Radix Ginseng 20 g,Angelica 15 g,Lily 15 g,Pear peal 20 g,Sanqi powder( red) 6 g,Centipede powder( red) 3 g,Scorpion powder Red) 3 g,Curcuma 15 g,Hedyotis diffusa 12 g,Scutellaria barbata 10 g,and Licorice 9g. The levels of syndromes,serum tumor markers and quality of life of patients were compared between the two groups before and after treatment. Results: Compared with that before treatment,the TCM syndrome scores of both groups after treatment have significantly improved( P < 0. 05) and that of the observation group was significantly better than that of the control group( P < 0. 05).The levels of CA125,CEA and Cyfra21-1 in both groups were significantly lower after treatment than that before treatment respectively( P < 0. 05). All of these indexes of the observation group were significantly better than that of the control group respectively( P < 0. 05). The stability rate of life expectancy in the observation group was 77. 5% and that in the control group was 55. 0%.The difference between the two groups in that was statistically significant( P < 0. 05). Conclusion: Traditional Chinese medicine Yifei Xiaoji Prescription in treatment of advanced non-small cell lung cancer can improve the symptoms,reduce the level of tumor markers and is effective to help achieve the stable quality of life of patients.
