理肝和脾法治疗难治性突发性聋的疗效观察

摘要
目的比较理肝和脾法指导下针药合用治疗"难治性ISHL"患者的临床疗效。方法将2015年4月～2016年12月在成都中医药大学附属医院、成都市第一人民医院和四川省八一康复中心收集到的420例"难治性ISHL"患者,按中央随机编号将其分在三个受试组,脱落14例,共计406例纳入统计。其中试验组为针药合用,共计138例168耳,对照组1为针刺组135例156耳,对照组2为中药组133例164耳,分别在病人治疗第0天,治疗2周后和治疗4周后三个时间点,对三个受试组患者耳聋疗效进行评定,并随访4周。观察各组听力的疗效差异。用Spss 22.0软件进行统计学分析。结果理肝和脾法指导下针药合用组对"难治性ISHL"患者听力有效率比针刺组和中药组高,其中经过常规治疗2周无效的"难治性ISHL"患者针药合用组听力有效率明显比针刺组和中药组高,未经治疗病程在3个月以上的"难治性ISHL"患者针药合用组听力有效率比针刺组和中药组高。结论理肝和脾法指导下针药合用治疗"难治性ISHL"安全有效。
Objective To compare the clinical efficacy under the treatment of the Smooth of the Liver and the Harmony of the Spleen for refractory idiopathic sudden hearing loss. Methods From April 2015 to December 2016 in Chengdu University of Traditional Chinese Medicine, the First People's Hospital of Chengdu and Rehabilitation Center of Sichuan province, 406 cases with refractory idiopathic sudden hearing loss collected were randomly assigned into three subjects according to the random number with the central machine. The experimental group was a combination of acupuncture and Chinese herbs, a total of 138 people with 168 ears.The control group 1 was the acupuncture group, 135 people with 156 ears, while the control group 2 was the Chinese herbs group, 133 people with 164 ears. Patients with deafness were evaluated in the three time points of zeroth days of treatment, after 2 weeks of treatment and 4 weeks of treatment, and were followed up for 4 weeks. Statistical analysis was performed using Spss 22.0 software to observe the difference in the efficacy of hearing. Results The treatment of the smooth of the liver and the harmony of the spleen was more effective for patients with refractory idiopathic sudden hearing loss, and the effective rate of the group that combined acupuncture and Chinese herbs was higher than that of the acupuncture group and the chinese herbs group. Conclusion The treatment of the Smooth of the Liver and the Harmony of the Spleen for refractory idiopathic sudden hearing loss is safe and effective.
