摘要
目的观察早期乳腺癌患者在超声诊断中的血流动力学表现特征,并就合并EB病毒感染患者对BI-RADS分级的影响及与乳腺癌发生的相关性进行分析。方法选取医院2016年8月-2018年1月收治确诊的68例早期乳腺癌患者为研究对象,同时选择60例乳腺良性病变患者进行对照研究,观察良、恶性肿瘤患者彩色多普勒血流显像(CDFI)血流信号差异,分析乳腺癌患者EB病毒感染情况以及对BI-RADS分级的影响及与乳腺癌发生的相关性。结果乳腺癌患者与乳腺良性病变患者胸廓动脉血流动力学各项参数比较均无统计学意义;研究组患者血流信号丰富,CDIF血流分级组间检验结果差异有统计学意义(P<0.05);68例乳腺癌患者中检出EB病毒感染19例,良性病变患者中未检出,比较差异有统计学意义(P<0.05);EB病毒与BI-RADS分级无明显相关,合并EB感染患者多为BI-RADS4/5级。结论多普勒超声能够有效鉴别乳腺良、恶性病变的血流情况,测定血流动力学参数可作为主要诊断标准,且合并EB感染患者多为BI-RADS高分级患者,但无明显相关性。
        OBJECTIVE To observe the hemodynamic performance of ultrasonic diagnosis of the patients with early breast cancer and observe the influence of Epstein-Barr(EB)virus infection on BI-RADS classification and its correlation with breast cancer so as to provide guidance for clinical diagnosis and treatment.METHODS From Aug2016 to Jan 2018,totally 68 patients with early breast cancer who were treated in the hospital were recruited as the study objects,meanwhile,60 patients with benign breast lesions were chosen as the control group.The blood flow signals were observed and compared between the patients with benign lesions and the patients with malignant tumors,and the prevalence of EB virus infection,influence on the BI-RADS classification and its correlation with breast cancer were analyzed.RESULTS There were no significant differences in the parameters of thoracic artery hemodynamics between the patients with breast cancer and the patients with benign breast lesions.The patients of the study group had abundant blood flow signals,and there was significant between-group difference in the CDIF blood flow classification(P<0.05).Of the 68 patients with breast cancer,19 had EB virus infection,while none of the patients with benign lesions had EB virus infection,and there was significant difference(P<0.05).The EB virus was not remarkably associated with the BI-RADS classification,and the majority of the patients complicated with EB virus infection was grade BI-RADS4/5.CONCLUSION Doppler ultrasound can effectively distinguish the blood flow between the benign lesions and malignant tumors,the hemodynamic parameters can be used as the major diagnostic criteria,the majority of the patients complicated with EB virus infection are the high grade BIRADS,however,there was no significant correlation.
