摘要
目的:探讨多层螺旋CT重建后处理技术在肺部磨玻璃小结节定性诊断中的应用价值。方法:收治肺内磨玻璃结节患者45例,通过多层螺旋CT扫描以及多平面重建技术、最大密度投影法、容积成像法处理图像以后,对处理的技术情况进行分析。结果:恶性病变患者纯磨玻璃样结节与混合磨玻璃样结节的诊断差异具有统计学意义。边缘形态中VR、MIP和MPR相比有差异。任何单一诊断在边缘形态、邻近结构改变方面都有差异。在恶性病变中,VR、MIP的边缘形态、形状、瘤肺界面和MPR有差异。在边缘形态、形状、瘤肺界面、邻近结构改变中3者联合与任何单一诊断均有差异,P均<0.05。多层螺旋CT处理技术应用后对肺部磨玻璃小结节疾病的诊断准确性由好到坏分别为三者联合、多平面重建技术、最大密度投影法、容积成像法。结论:采用多层螺旋CT扫描并结合MPR、VR、MIP重建后处理技术对肺部磨玻璃小结节早期诊治有较高的临床应用价值。
Objective:To evaluate the value of CT multiplanar reconstruction technique in qualitative diagnosis of pulmonary ground glass nodule.Methods:45 patients with pulmonary ground glass nodule were selected.The processing technology was analyzed by multi slice spiral CT scanning and multiplanar reconstruction,maximum density projection and volume imaging.Results:The diagnostic value of pure ground glass nodules and mixed glass like nodules in malignant lesions was statistically significant.The differences of VR,MIP and MPR were different in the edge morphology.There were differences in the shape of the edge and the change of adjacent structure in any single diagnosis.In malignant lesions,the VR, MIP edge shape,shape,tumor lung interface and MPR were different.There were differences of three groups in the edge morphology,shape,tumor lung interface,adjacent structural changes,all P<0.05.The accuracy of multi-slice spiral CT in the diagnosis of nodular disease in the lungs from good to bad was:the three joint,multiplanar reconstruction,maximum density projection,volume imaging.Conclusion:Multi slice spiral CT scanning combined with MPR,VR and MIP reconstruction technique has a high clinical value in the early diagnosis and treatment of small pulmonary nodules.
