血浆D-二聚体及凝血四项检测在妊娠高血压综合征中的价值

摘要
目的探讨血浆D-二聚体(D-D)及凝血四项检测在妊娠期高血压综合征孕妇中的应用价值。方法采用回顾性分析的方法分析2015年1月~2016年6月在本院诊断为妊娠期高血压综合征的87例孕妇(妊娠高血压综合征组)、30例正常孕妇(正常妊娠组)及30例未孕妇女(正常对照组)的资料,比较各组的凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)、D-D水平。结果妊娠高血压综合征组的PT、APTT明显短于正常妊娠组和正常对照组,FIB、D-D水平明显高于正常妊娠组和正常对照组,差异有统计学意义(P<0.05)。正常妊娠组的PT、APTT明显短于正常对照组,FIB、D-D水平明显高于正常对照组,差异有统计学意义(P<0.05)。三组的TT水平比较,差异无统计学意义(P>0.05)。结论妊娠期妇女的血液处于高凝状态,尤其是妊娠高血压综合征妇女,因此常规检查凝血功能和D-D有助于了解妊娠高血压综合征妇女的凝血及纤溶功能情况,对辅助妊娠期高血压综合征的诊断、病情观察、治疗及预防其并发症具有重要临床意义。
Objective To investigate the value of plasma D-dimer(D-D) and coagulation four markers in pregnant women with pregnancy-induced hypertension syndrome.Methods A retrospective analysis was carried out on 30 cases of normal pregnant women(the normal pregnancy group),30 cases of non-pregnant women(the normal control group) and87 cases of pregnant women who were diagnosed as pregnancy-induced hypertension syndrome(the pregnancy-induced hypertension group) in our hospital from January 2015 to June 2016.The level of prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),fibrinogen(FIB) and D-D level were compared.Results The PT and APTT of the pregnancy-induced hypertension group was significantly shorter than that of the normal pregnancy group and the normal control group,and the level of FIB and D-D of the pregnancy-induced hypertension group was significantly higher than that of the normal pregnancy group and the normal control group,with significant difference(P<0.05).The PT and APTT of the normal pregnancy group was significantly shorter than that of the normal control group,and the level of FIB and D-D of the normal pregnancy group was significantly higher than that of the the normal control group,with significant difference(P<0.05).There was no statistical difference between the TT of the three groups(P>0.05).Conclusion The blood of pregnant women is in hypercoagulable state,especially in pregnancy-induced hypertension women.Therefore,the routine examination of blood coagulation and D-D contributes to the understanding the coagulation and fibrinolysis function of pregnancy-induced hypertension women.It has important clinical significance in the diagnosis,observation,treatment and prevention of complications of pregnancy-induced hypertension syndrome.
