护理干预对膝关节置换术后止血带局部反应的效果

摘要
目的观察膝关节置换术后止血带局部反应护理干预的临床效果。方法选取2015年3月—2018年4月膝关节置换术后出现止血带局部反应的80例患者随机分为两组进行研究,其中40例接受常规护理(对照组),另40例接受中药热奄包及冰敷护理干预(研究组),比较护理干预效果。结果护理干预前两组间VAS评分差异无统计学意义(P> 0.05);护理后3 d、5 d时,研究组VAS评分低于对照组,差异有统计学意义(P <0.05)。比较自主活动恢复时间,研究组踝关节背伸并直腿抬高运动、股四头肌等长收缩运动、踝泵运动时间均早于对照组,数据差异有统计学意义(P <0.05)。研究组护理满意度明显高于对照组(90.0%vs. 52.5%,P <0.05)。结论膝关节置换术后止血带局部反应接受中药热奄包外敷及冰敷护理干预,可加快患者肢体功能恢复,明显缓解疼痛。
Objective To observe the clinical effect of nursing intervention for local reaction of tourniquet after knee arthroplasty.Methods 80 cases of patients with local tourniquet reaction after knee arthroplasty from March 2015 to April 2018 were randomly divided into two groups. 40 cases were received routine nursing(control group), and another 40 cases were received nursing intervention of hot embalming and ice compress of traditional Chinese medicine(study group). Then,the effect of nursing intervention were compared. Results There was no significant difference in VAS score between the two groups before and after nursing intervention(P > 0.05); After nursing for 3 and 5 days, the VAS score in the study group was lower than that in the control group,and the difference was statistically significant(P < 0.05). The recovery time of autonomous activities were compared, and the time of ankle back extension and straight leg elevation, quadriceps femoris isometric contraction and ankle pump exercise in the study group was earlier than those in the control group, and the dif ference was statistically significant(P < 0.05). The nursing satisfaction in the study group was significantly higher than that in the control group(90.0% vs. 52.5%, P < 0.05).Conclusion The nursing intervention of traditional Chinese medicine encapsulated dry-hot external treatment and ice compress for local reaction of tourniquet after knee arthroplasty which can accelerate the recovery of limb function and relieve pain obviously.
