河南省艾滋病患者手术疾病谱变化及手术需求现状分析

摘要
目的探讨河南省艾滋病患者手术疾病谱的变化及手术需求现状,为艾滋病患者的手术治疗提供依据。方法采用回顾性分析方法,将河南省传染病医院2010—2015年住院的艾滋病手术患者纳入调查,内容包括性别、年龄、学历、婚姻、合并其他感染性疾病、手术疾病分科和手术疾病分类、患者对手术需求满足度等,分析艾滋病手术疾病谱变化和治疗需求现状。结果 2010—2015年接受艾滋病手术治疗的患者共1 754例,涉及外科手术1 076例(61.3%),妇产科348例(19.8%);外科以行肿块(良性)、肿瘤(恶性)切除术381例为主,占外科手术的35.4%;妇产科行剖宫产术154例,占妇产科手术的44.3%,肿块(良性)、肿瘤(恶性)切除术156例,占其44.8%,两项基本持平。外科手术中,除肿块肿瘤切除、结核病灶清除、骨折修复手术外,余手术涉及人体各个系统疾病,其中以消化系统为主,共295例,非消化系统189例,二者比例为1.6∶1。2014年艾滋病手术需求满足度、服务满意度和效果满意度分别为70.0%(35/50)、82.0%(41/50)和76.0%(38/50),2015年分别为92.0%(46/50)、100.0%(50/50)和94.0%(47/50)。2015年艾滋病手术需求满足度、服务满意度和效果满意度均高于2014年,差异有统计学意义(P<0.05)。2014年艾滋病患者手术后伤口不愈合发生率、肺部感染发生率、职业暴露发生率分别为3.6%、4.5%、2.4%,2015年分别为1.6%、1.0%、0.6%,均较2014年降低。结论河南省艾滋病患者手术疾病谱逐渐扩大,手术需求满足度得到提升。
Objective To explore the change of surgical disease spectrum and the current situation of surgical needs of AIDS patients in Henan Province,and to provide the basis for the surgical treatment of some AIDS patients. Methods A retrospective analysis study was conducted in the Henan Infectious Disease Hospital from January of 2010 to December of 2015. Inpatient surgery patients with AIDS were included in this study. The information of gender,age,education level,marriage status,other complicated infectious diseases, surgery branches with disease classification,and the satisfaction with the demand for surgery were collected. The changes of AIDS surgical disease spectrum,the current situation of surgery needs for AIDS patients and the challenges and prospects of the surgical risks were analyzed. Results A total of 1 754 AIDS patients received surgery treatment from 2010 to 2015,including 1 076 cases( 61. 3%) with the surgical operation and 348 cases( 19. 8%) of gynecology and obstetrics. 381 cases of mass( benign) and tumor( malignant) resection were performed in surgery,accounting for 35. 4% of surgical operations. In obstetrics and gynecology,154 cases of cesarean section performed,accounting for 44. 3% of operations,and there was 156 cases of mass( benign) and tumor( malignant) resection there,accounting for 44. 8% of operations. In surgery operations,besides the tumor resection,tuberculosis kitchen cleared and fracture repair surgery,operations involved a variety of systemic diseases in the body. Among them,the digestive system accounted for 295 cases,rather than 189 cases in other systems,with a ratio of 1. 6 to 1. The patients' satisfactions of need,service and effect of AIDS surgery were 70%( 35/50),82%( 41/50) and 76%( 38/50) respectively in 2014,and 92%( 46/50),100%( 50/50) and 94%( 47/50) respectively in 2015. The satisfactions of need,service and effect of AIDS surgery in 2015 were higher than those in 2014,and the differences were statistically significant( P < 0. 05). The incidence rates of wound nonunion,pulmonary infection after HIV/AIDS surgery and occupational leakage were 1. 6%,1. 0% and 0. 6% respectively in2015,which were 3. 6%,4. 5% and 2. 4% respectively in 2014. Conclusion The surgical disease spectrum of AIDS patients has gradually expanded in Henan Province,and the satisfaction of operation need is improved.
