摘要
目的:评价桑菊清解汤对大鼠单肺通气肺损伤的影响。方法:健康成年雄性SD大鼠48只,体重300~350g,随机分为4组(n=12):对照组(C组)、双肺通气组(S组)、单肺通气组(D组)和桑菊清解汤组(SJ组)。采用大潮气量通气建立肺损伤模型,D组和SJ组气管插管行右肺通气1h,潮气量5mL/kg,频率80次/min,I∶E 1∶2;C组行气管插管自主呼吸。采用桑菊清解汤300g灌胃,1次/d,于第10次灌胃后2h时开始制备模型;C组、S组及D组给予等容量生理盐水。于机械通气前(T0)、机械通气结束时(T1)、机械通气后30min(T2)时采集动脉血样,检测动脉血气,测定呼吸指数(RI)和氧合指数(OI)后处死大鼠,取肺组织,ELISA法检测肺组织TNF-a、IL-6和IL-10的含量,肺组织湿/干重比(W/D)和HO-1表达,观察肺组织病理学改变。结果:与C组比较,D组和SJ组肺组织TNF-a、IL-6、IL-10含量及W/D比升高,HO-1表达上调(P<0.05),S组各指标差异无统计学意义;与S组比较,D组和SJ组肺组织HO-1表达上调,TNF-a、IL-6、IL-10含量及W/D比升高,RI升高,OI降低(P<0.05)。与D组比较,SJ组肺组织、IL-10含量升高,TNF-a、IL-6含量及W/D比降低,HO-1表达上调,RI降低、OI升高(P<0.05)。结论:桑菊清解汤可通过上调肺组织HO-1的表达进而减轻大鼠单肺通气所致肺损伤。
        Objective:Investigate the effects of sangju qingjie decoction on lung tissues injury induce by one lung ventilation rats.Methods:Forty-eight SD rats of both sexes weighing 300~350 g were randomly divided into four groups(n=12 each):The control group(group C),the two lung ventilation group(group T),one lung ventilation group(group O) and the Sangju Qingjie Decoction preconditioning group(group SJ). The animals were anesthetized with intraperitoneal pentobarbital sodium 40 mg/kg.In group T,the animals were performed for 1 h(Vt 5 mL/kg,RR88 bpt/min,I ∶E 1 ∶2),In groups O and SJ,the animals were intubated and OLV was performed for 1 h(Vt 5 mL/kg,RR 80 bpt/min,I∶E 1∶2).In group SJ,Sangju Qingjie Decoction 300 g was injected into the stomach through a gastric tube once a day for 10 consecutive days,and mechanically ventilation was performed 2 h after the last injection. while in groups T,O and C,the equal volume of normal saline was given instead of Sangju Qingjie Decoction. Arterial blood samples were collected respectively at the initial of the experiment(T0),0 min after ventilator(T1)and 30 min after ventilator(T2),the alveolararterial oxygen pressure difference(PA-aDO2),respiratory index(RI),were measured with artery blood gas analysis. the lung were removed for determination of the contents of TNF-a,IL-6 and IL-10,the expression of heme oxygenase-1. The W/D lung weight ratio was calculated. Results:RI was significantly increased and OI was decreased at T1 and T2 as compared with the baseline T0 in group O and group SJ; TNF-aα,IL-6 and IL-10 concentrations were significantly higher and W/D was significantly higher in group O and group SJ than in group C; TNF-α,IL-6 concentrations and W/D were significantly lower in group SJ than group O,but IL-10 concentrations was significantly higher in group SJ than in group O. Conclusion:Sangju qingjie decoction preconditioning can attenuate lung injury induced by one lung ventilation in rats,Inhibition of heme oxygenase-1 expression is involves in the mechanism.
