治疗性沟通配合心理疏导对冠心病合并心绞痛患者负性情绪、遵医行为及日常生活质量的影响

摘要
目的:探究治疗性沟通配合心理疏导对冠心病(CHD)合并心绞痛患者负性情绪、遵医行为及日常生活质量的影响。方法:纳入2017年2月-2018年2月我院收治的100例CHD合并心绞痛患者,按信封法随机分为观察组与对照组,各50例。对照组治疗过程中配合心理疏导,观察组在对照组上联合治疗性沟通干预,比较两组临床疗效、焦虑自评量表(SAS)、抑郁自评量表(SDS)评分、遵医行为及西雅图心绞痛量表(SAQ)评分。结果:观察组临床疗效总有效率90.00%(45/50)显著高于对照组72.00%(36/50),遵医行为遵从率92.00%(46/50)显著高于对照组74.00%(37/50),差异均有统计学意义(χ~2=5.263,5.741;P<0.05);干预后观察组SAS、SDS评分显著低于对照组,SAQ评分显著高于对照组,差异亦有统计学意义(t=2.214,2.198,5.141,3.353,2.928,5.712,2.721;P<0.05)。结论:治疗性沟通配合心理疏导可显著改善CHD合并心绞痛患者负性情绪,提高患者遵医行为及日常生活质量,可考虑在临床治疗中推广应用。
Objective:To explore the effects of therapeutic communication combined with psychological counseling on negative emotions,compliance behaviors and quality of daily life in patients with coronary heart disease(CHD)complicated with angina pectoris.Methods:A total of 100 cases of patients with CHD complicated with angina pectoris admitted to our hospital from February 2017 to February 2018 were included and randomly divided into observation group and control group according to the envelope method,with 50 cases in each group.Control group was assisted with psychological counseling during treatment,and observation group was combined with therapeutic communication intervention based on control group.The clinical efficacy,self-rating anxiety scale(SAS)score,self-rating depression scale(SDS)score,compliance behaviors and Seattle Angina Questionnaire(SAQ)score were compared between the two groups.Results:The total effective rate of clinical efficacy in observation group was significantly higher than that in control group,and the compliance rate of compliance behaviors was significantly higher than that in control group(χ~2=5.263,5.741;P<0.05).After intervention,the scores of SAS and SDS in observation group were significantly lower than those in control group while the SAQ score was significantly higher than that in control group(t=2.214,2.198,5.141,3.353,2.928,5.712,2.721;P<0.05).Conclusion:Therapeutic communication combined with psychological counseling can significantly improve negative emotions in CHD patients with angina pectoris,and improve patients' compliance behaviors and quality of daily life,and it can be considered for promotion and application in clinical treatment.
