COX-2、VEGF及miRNA表达水平与食管鳞状细胞癌患者的临床疗效及预后的关系

摘要
目的探究COX-2、VEGF及miRNA表达水平与食管鳞状细胞癌患者的临床疗效及预后的关系。方法随机选择120例临床资料完整的食管鳞状细胞癌患者作为观察组;并选择同期体检的120例正常人为对照组。食管鳞状细胞癌患者均采用根治性切除术治疗,治疗后并按患者治疗效果分为有效组和无效组。记录并对比患者的COX-2、VEGF及miRNA表达水平,并对比原发病灶和复发转移病灶间COX-2、VEGF与miRNA阳性表达率。结果观察组患者的COX-2、VEGF、miRNA表达量高于对照组(P <0. 05)。对比发现,食管鳞状细胞癌COX-2、VEGF及miRNA表达量与患者的年龄、性别及组织分化程度有轻微关系(P> 0. 05)。无效组的COX-2、VEGF、miRNA表达量明显高于有效组(P <0. 05)。食管鳞状细胞癌患者原发病灶的COX-2、VEGF与miRNA阳性表达率明显低于复发转移病灶患者(P <0. 05)。结论监测COX-2、VEGF及miRNA表达水平,能有效预测食管鳞状细胞癌的临床疗效及预后情况,值得临床借鉴。
        Objective To investigate the relationship between the expression of COX-2,VEGF and miRNA and the clinical efficacy and prognosis of patients with squamous cell carcinoma of the esophagus. Methods 120 cases of complete clinical data of patients with esophageal squamous cell carcinoma were selected as the observation group from January 2014 to January2016 in our hospital diagnosed; and select the same period to the hospital physical examination of 120 normal people as control group. Patients with esophageal squamous cell carcinoma were treated with radical resection. After treatment,the patients were divided into effective group and ineffective group according to the treatment effect. The expression levels of COX-2,VEGF and miRNA were recorded and compared,and the positive rates of COX-2,VEGF and miRNA were compared between primary lesions and recurrent lesions. Results The levels of COX-2 and VEGF in the observation group were significantly higher than those in the control group,and the expression level of miRNA was lower than that of the control group( P < 0. 05). In contrast,the expression of COX-2,VEGF and miRNA in esophageal squamous cell carcinoma was slightly correlated with age,sex and histological differentiation( P > 0. 05). The COX-2 and VEGF in the ineffective group were significantly higher than those in the control group,and the expression level of miRNA was significantly lower than that in the control group( P < 0. 05). The positive rates of COX-2,VEGF and miRNA in primary lesions of esophageal squamous cell carcinoma patients were significantly lower than those in patients with recurrence or metastasis( P < 0. 05). Conclusion Monitoring the expression level of COX-2 and VEGF combined with miRNA can effectively predict the clinical efficacy and prognosis of esophageal squamous cell carcinoma,and it is worthy of clinical reference.
