Tei指数对慢性肾病患者心功能早期评估意义分析

摘要
目的:分析Tei指数对慢性肾病(Chronic Kidney Disease,CKD)患者心功能早期评价的意义。方法:收集早期CKD患者125例纳入研究组,其中CKD1期55例,CKD2期70例;收集同期健康人群60例纳入对照组。利用组织多普勒(TDI技术)测出所有纳入研究人员的左右心室相关指标,再分别计算出各组Tei指数进行各自对比分析。结果:研究组与对照组对象的左心室Tei指数依次是:0.47±0.11、0.38±0.05,右心室Tei指数依次为:0.41±0.09、0.29±0.07,研究组高于对照组,有统计学差异(P<0.05),提示CKD患者的Tei指数较正常人群偏高,即CKD患者早期左右心室整体功能开始出现下降趋势;CKD1期、2期患者的左心室Tei指数依次是:0.43±0.07、0.51±0.09,右心室Tei指数依次为:0.39±0.06、0.43±0.07,CKD2期高于CKD1期,有统计学差异(P<0.05),提示随着患者CKD分期的增加,其左右心室Tei指数逐渐增大,提示整体心功能逐渐下降。结论:组织多普勒Tei指数是综合评价心功能的一项敏感指标,可早期发现CKD1期、2期患者整体心功能的变化情况,并且随着患者CKD分期的增加,患者整体心功能逐渐下降。
Objective:To analyze the significance of Tei index in early evaluation of cardiac function in patients with chronic kidney disease(CKD).Methods:125 patients with early CKD were enrolled, with 55 cases of CKD1 assigned in the study group and 70 cases with CKD2 assigned in the control group. 60 healthy subjects were included in the control group. All the left and right ventricle-related indicators of the subjects involved were measured by tissue Doppler imaging(TDI technology), and the Tei indices in the two groups were calculated and compared.Results:The left ventricular Tei index in the study group and the control group were 0.47±0.11 and 0.38±0.05, and the right ventricular Tei index was 0.41±0.09 and 0.29±0.07, respectively, with indices in the study group higher than the control group, and there were statistical differences between the two groups(P<0.05), which suggested that the Tei index of patients with CKD was higher than that in normal population, that was to say, the overall left and right ventricular function of CKD patients began to decline. The left ventricular Tei index of patients with stage CKD1 and stage CKD1 was 0.43±0.07 and 0.51± 0.09, and the right ventricular Tei index was 0.39±0.06 and 0.43±0.07, and indices of patients with CKD2 were higher than those of patients with CKD1, with statistically significant difference(P<0.05), which suggested that the left and right ventricular Tei indices increased gradually with the increase of CKD staging, and that the overall heart function was gradually decreasing.Conclusion:Tissue Doppler Tei index is a sensitive index for comprehensive evaluation of cardiac function. It can detect the changes of global cardiac function in patients with CKD1 and CKD2, and the overall cardiac function of patients decreases with the increase of CKD staging.
