2016—2017年北京市海淀区14家二级医院抗菌药物应用分析

摘要
目的:了解北京市海淀区所有二级医院抗菌药物的使用情况。方法:分别统计2016年、2017年北京市海淀区14家二级医院抗菌药物的使用数据,对门急诊、住院患者抗菌药物使用强度、使用率以及病原送检率等相关指标进行统计分析。结果:2016年、2017年,北京市海淀区14家二级医院门诊抗菌药物使用率分别为9.8%、10.7%,门诊抗菌药物注射剂使用率分别为1.6%、1.2%;急诊抗菌药物使用率分别为26.5%、31.0%,急诊抗菌药物注射剂使用率分别为11.3%、11.0%;住院患者抗菌药物使用率分别为47.8%、50.3%,住院患者特殊使用级抗菌药物使用率分别为2.5%、2.4%;接受抗菌药物治疗的住院患者微生物送检率分别为47.0%、56.2%,接受限制使用级抗菌药物治疗的住院患者微生物送检率分别为61.5%、66.4%,接受特殊使用级抗菌药物治疗的住院患者微生物送检率分别为89.8%、92.2%;Ⅰ类切口手术围术期抗菌药物预防性使用率分别为48.7%、48.0%,其中,术前0.5~2 h给药率分别为92.0%、90.5%,预防用药疗程≤24 h的Ⅰ类切口手术所占比例分别为32.3%、37.3%,上述差异均无统计学意义(P>0.05)。另外,2016年、2017年住院患者抗菌药物使用强度分别为36.7、38.6 DDDs/(100人·d),住院患者特殊使用级抗菌药物使用强度分别为2.4、2.4 DDDs/(100人·d)。结论:经过抗菌药物临床应用专项整治,北京市海淀区二级医院抗菌药物的使用基本合理,相关指标趋于平稳。
        OBJECTIVE: To investigate the application of antibiotics in the secondary hospitals in Haidian District of Beijing during 2016-2017. METHODS: Data on the application of antibiotics in 14 secondary hospitals in Haidian District of Beijing in 2016 and 2017 were separately counted, and the relevant indicators such as the application intensity of antibiotics, the application rate and the detection rate of pathogens for outpatient, emergency and inpatient patients were analyzed. RESULTS: In 2016 and 2017, the application rate of antibiotics in outpatient departments of 14 secondary hospitals in Haidian District of Beijing was 9.8% and 10.7%, respectively, and the application rate of antibiotics in outpatient departments was 1.6% and 1.2%, respectively. The application rate of antibiotics in emergency department was 26.5% and 31.0%, respectively, and that of antibiotics injection in emergency department was 11.3% and 11.0%, respectively. The application rate of antibiotics in inpatients was 47.8% and 50.3%, respectively, and that of inpatients with special use grade antibiotics was 2.5% and 2.4%, respectively. The detection rate of microorganism of inpatients receiving antibiotics was 47.0% and 56.2%, respectively. The detection rate of microorganism of inpatients receiving restricted-use antibiotics was 61.5% and 66.4%, respectively. The detection rate of microorganism of inpatients receiving special use grade antibiotics was 89.8% and 92.2%, respectively. The preventive application rate of antibiotics in type I incision surgery during perioperative period was 48.7% and 48.0%, respectively, and the administration rate before surgery of 0.5-2 h was 92.0% and 90.5%, the proportion of type I incision surgery with a course of prophylaxis of ≤24 h was 32.3% and 37.3%, respectively. And the above differences were not statistically significant(P>0.05). In addition, in 2016 and 2017, the application intensity of antibiotics in inpatients was 36.7 and 38.6 DDDs/(100 people·d), and the application intensity of special use grade antibiotics in inpatients was 2.4 and 2.4 DDDs/(100 people·d). CONCLUSIONS: After the special rectification of the clinical application of antibiotics, the application of antibiotics in the secondary hospital of Haidian District of Beijing is basically reasonable, and the relevant indicators tend to be stable.
