摘要
目的探讨vimentin(波形蛋白)表达水平与肛瘘感染细菌谱的关系,以及肛瘘标本中vimentin表达水平与患者临床特征的关系。方法回顾性分析2017年1月至2017年12月杭州市第三人民医院肛肠科住院的120例肛瘘患者临床资料,采集患者瘘管分泌物进行细菌培养,瘘管组织vimentin表达水平采用免疫组化方法检测,分析细菌谱与vimentin表达水平关系,并分析vimentin表达水平主要部位及其与患者临床特征的关系。结果在120例肛瘘患者中,瘘管分泌物细菌培养结果以大肠埃希菌(63.3%)为主,其次为肺炎克雷伯菌(18.3%),金黄色葡萄球菌(7.5%)。vimentin表达水平在大肠埃希菌组与肺炎克雷伯菌组、金黄色葡萄球菌组及其他细菌组之间相比较,差异有统计学意义(P <0.05)。高位肛瘘组vimentin表达水平低于低位肛瘘组,复杂性肛瘘组vimentin表达水平低于单纯性肛瘘组,差异均有统计学意义(均P <0.05),性别、年龄和内口方向与vimentin表达水平无关(均P> 0.05)。结论肛瘘感染以肠源性细菌感染为主,以大肠埃希菌最常见,vimentin表达水平与肛瘘位置与病变程度有关,且vimentin可能参与了肛瘘的发生发展过程。
        Objectives To investigate the relationship between vimentin expression and bacterial spectrum of anal fistula infection and the relationship between vimentin expression and clinical characteristics of patients. Methods This was a retrospective analysis of clinical data of 120 hospitalized patients with anal fistula treated at the Department of Anal and Intestinal Surgery,The Third Hangzhou People's Hospital, between January 2017 and December 2012. Bacterial spectrum of anal fistula infection was analyzed using bacteria culture of secretion of fistula. The expression of vimentin in fistula tissue was analyzed using immunohistochemical method. We analyzed the relationship between bacterial spectrum and vimentin expression and the relationship between vimentin expression and clinical characteristics of patients. Results Among the 120 patients with anal fistula, culture showed the main bacteria were Escherichia coli(63.3%), Klebsiella pneumoniae(18.3%) and Staphylococcus aureus(7.5%). Vimentin expression was significantly different among Escherichia coli, Klebsiella pneumoniae, Staphylococcus aureus and other bacteria(P < 0.05). Vimentin expression was significantly lower in patients with high fistula than with low fistula, and significantly lower in complex fistula than in simple fistula(P < 0.05). Sex, age, and direction of internal opening of fistula did not affect vimentin expression(P > 0.05). Conclusion Bacterial infection in fistula were mostly intestinal bacteria, with Escherichia coli being the most common. The expression of vimentin is correlated with the location and severity of fistula. It was possible that vimentin contributes to the occurrence and development of fistula.
