摘要
目的探讨小量多次补充肺表面活性物质(PS)治疗婴幼儿急性呼吸窘迫综合征(PARDS)的有效性。方法 96例确诊为PARDS的患儿,随机分为基础组、对照组和观察组。三组患儿均接受相应的综合治疗,对照组和观察组分别另外补充单剂常量(200mg/kg)和多剂小量(每次100mg,共4次)的PS。比较各组患儿治疗前后的血气分析指标、机械通气参数及不良反应率的差异。结果患儿经治疗24h后血气分析指标均有不同程度改善,机械通气参数下调,病情缓解。观察组疗效最显著,其次为对照组,三组间比较差异有统计学意义(P<0.05)。各组患儿均未见明显不良反应。结论补充外源性PS有助于PARDS的病情改善,尤以小量多次给予效果更佳。
        Objective To explore the effectiveness of small amount of supplemental pulmonary surfactant(PS) in the treatment of acute respiratory distress syndrome(PARDS) in infants. Methods Ninety-six cases of PARDS were randomly divided into the basic group,the control group and the observation group.All three groups were treated with corresponding comprehensive treatment.the control group and observation group were supplemented with single dose constant(200 mg/kg) and multiple doses of small amount(100 mg each time,a total of 4 times) of PS respectively.The blood gas analysis indexes,mechanical ventilation parameters and adverse reaction rate of each group were compared before and after treatment. Results After the treatment of 24 h,the blood gas analysis indexes were improved in different degrees,the mechanical ventilation parameters were decreased,and the illness was relieved.The effect of observation group was the most significant,followed by the control group,the difference between the three groups was statistically significant(P < 0.05).No significant adverse reactions were found in all groups. Conclusion Supplement of exogenous PS could improve the condition of PARDS,especially the effect of small dose and multiple doses was better.
