略谈治温病运用调胃承气汤

摘要
调胃承气汤出自仲景《伤寒论》,主治太阳病误汗后的阳明结实,或伤寒直中阳明所致燥实,但气滞不甚之证。到后世温病学形成,下法在温病治疗中占据重要地位,《温病条辨》载有诸多应用下法方证,运用承气汤也有颇多创新。而温病之用又与伤寒之用有不同,讲究"下不厌早"之说,又忌辛温苦燥之品伤津,故多选调胃承气汤治疗阳明热结及兼证、下焦热瘀互结证,更有"以泻代清"之用。
Tiaowei Chengqi Decoction,from Treatise on Exogenous Febrile Diseases of ZHANG Zhongjing,has been used to treat the Yangming heat-evilaccumulating after the mistreatment of perspiration of the Taiyang disease,and the Yangming heat-evilaccumulating by typhoid feverdirect attacking,however Qi stagnation has not been verified completely. Until the formation of warm diseases in the later generations,purgative method whose many applications are recorded in Detailed Analysis of Epidemic Warm Diseases,has occupied an important position in the treatment of warm diseases,in which Chengqi Decoction has been innovated a lot. However,warm diseases are different from cold diseases,it focuses more on "purgative method not mind using early",and avoids impairment of fluid by warm-pungent and bitter-dryness drug. Because of that,to treat the Yangming heatevilaccumulating and accompanying symptoms always use Tiaowei Chengqi Decoction. To learn more,it also can treat stagnated heat in lower-Jiao. In addition,another significant function of Tiaowei Chengqi Decoction is"use purgative method substitute for clearing method".
