玻璃纤维桩联合BisCem树脂水门汀对残根残冠修复患者PGE_2、IL-1β水平的影响

摘要
目的探讨玻璃纤维桩联合Bis Cem树脂水门汀对残根残冠修复患者前列腺素E2(PGE2)、白细胞介素-1β(IL-1β)水平的影响。方法选取2015年10月~2017年6月南京明基医院口腔科残根残冠修复患者84例,根据随机数字表法分为对照组(n=42)与研究组(n=42)。两组均采取玻璃纤维桩联合树脂材料实施修复,研究组采取Bis Cem树脂水门汀,对照组采取PULPDENT双重固化树脂水门汀。统计并比较两组治疗前及治疗后1个月咀嚼效能(咬合力、咀嚼效率),治疗前及治疗后3 d、5 d龈沟液PGE2,IL-1β水平,治疗满意度,并发症发生率。结果治疗前两组咀嚼效率及咬合力间比较,差异均无统计学意义(P>0.05);治疗后1个月两组咀嚼效率及咬合力均较治疗前增大,且研究组大于对照组(P<0.05);治疗前两组龈沟液PGE2、IL-1β水平比较,差异无统计学意义(P>0.05);治疗后两组龈沟液PGE2、IL-1β水平较治疗前增高,且研究组低于对照组(P<0.05);研究组修复效果、舒适度、咀嚼能力、外观效果满意度分值均高于对照组(P<0.05);研究组并发症总发生率(9.52%)明显低于对照组(26.19%)。结论采取玻璃纤维桩联合Bis Cem树脂水门汀治疗残根残冠修复患者,利于改善其咀嚼效能,减少创伤程度,抑制龈沟液PGE2、IL-1β水平增高幅度,利于提高治疗满意度,且并发症发生率较低,安全性较高。
        Objective To investigate the effect of glass-fiber post combined with Bis Cem resin cement on levels of prostaglandin E2(PGE2) and interleukin-1β(IL-1β) of gingival crevicular fluid in patients with restoration of residue crown and residue root. Methods From October 2015 to June 2017, 84 patients with restoration of residue crown and residue root in Department of Stomatology of Nanjing Mingji Hospital were randomly divided into control group(n = 42)and study group(n = 42). The two groups were all repaired with glass-fiber post and resin material. The study group was treated with Bis Cem resin cement and the control group was treated with PULPDENT dual-curing resin cement.The masticatory efficacy(bite force, chewing efficiency) before treatment and after 1 month of treatment, the level of PGE2 and IL-1β in gingival crevicular fluid before treatment and after 3 d and 5 d of treatment, the satisfaction and the incidence of complications were statistically analyzed between the two groups. Results There was insignificant difference in masticatory efficiency and bite force between the two groups before treatment(P > 0.05). After 1 month of treatment, the masticatory efficiency and bite force of the two groups increased compared with those before treatment,and the study group was larger than the control group(P < 0.05). There was insignificant difference in levels of PGE2 and IL-1β of gingival crevicular fluid between the two groups before treatment(P > 0.05). After treatment, the levels of PGE2 and IL-1β of gingival crevicular fluid of the two groups increased, but those in the study group were lower than those in the control group(P < 0.05). The repair effect, comfort degree, masticatory ability and the satisfaction score of appearance effect of the study group were higher than those of the control group(P < 0.05). The total incidence of complications in the study group(9.52%) was lower than that in the control group(26.19%)(P < 0.05). Conclusion Glassfiber post combined with Bis Cem resin cement in treating patients with restoration of residue crown and residue root is beneficial to improve the masticatory efficacy, reduce the degree of trauma, and inhibit the increase of PGE2 and IL-1βlevels of gingival crevicular fluid, and improve the satisfaction of the treatment. Besides, the incidence of complications is lower and the safety is higher.
