新疆乡镇卫生院基本公共卫生服务开展现状

摘要
目的对新疆乡镇卫生院基本公共卫生服务的开展情况进行调查,针对评价结果中存在的问题,为政府制定相关政策提供建议。方法采用描述性分析,对新疆37所乡镇卫生院进行调查,运用秩和比法对新疆乡镇卫生院的基本情况进行评价。结果抽样地区乡镇卫生院的健康档案建档率均已达到70%以上,其中喀什地区的建档率达到90%左右;老年人的建档率达到90%左右;样本乡镇卫生院的儿童健康管理率多为70%～80%,只有巴州地区不足50%,差异具有统计学意义;儿童保健工作开展得较好;孕产妇健康管理率在65%～85%。南疆地区的健康档案建档率高于北疆地区;南北疆地区乡镇卫生院对重型精神病的管理较好,均符合标准;高血压和糖尿病规范管理率略低于标准,这两项工作开展欠佳;大部分乡镇卫生院在地方病与慢性非传染性疾病管理制度的建立上,都缺少心血管疾病的管理制度,对心血管疾病的管理工作还不够重视。结论政府需加强领导,进一步加大对乡镇卫生院的投入,改善农村基本设施条件,积极推动乡镇卫生院基本公共卫生服务的开展工作;加强对基层卫生人力资源的培养,以提高乡镇卫生院基本公共卫生服务能力。
Objective To study the development of basic public health services in Xinjiang township health centers and to provide suggestions for the government to formulate relevant policies in view of the problems existing in the evaluation results.Methods Descriptive analysis was used to investigate 37 township health centers in Xinjiang, and rank sum ratio method was used to evaluate the basic situation of township hospitals in Xinjiang.Results The establishment rate of health archives in township health centers in the sampling area reached more than 70%, including 90% in Kashi and 90% in the elderly.The health management rate of children in the sampled township health centers was mostly 70%-80%, only in Bazhou area. The difference was statistically significant. The child health care was well carried out, and the maternal health management rate was about 65-85%.The establishment rate of health archives in southern Xinjiang was higher than that in northern Xinjiang; the management of severe mental illness in township health centers in northern and southern Xinjiang was better, and the standardized management rate of hypertension and diabetes mellitus was slightly lower than that in north and south Xinjiang, and these two tasks were not well carried out.Most township hospitals lacked the management system of cardiovascular diseases in terms of management of endemic diseases and chronic non-communicable diseases, and did not pay enough attention to the management of cardiovascular diseases.Conclusion The government needs to strengthen its leadership and further increases its commitment to the township. The investment of township health centers can improve the conditions of rural basic facilities, actively promote the development of basic public health services of township health centers, and strengthen the cultivation of basic health human resources in order to improve the capacity of basic public health services of township hospitals.
