广泛型侵袭性牙周炎患者龈沟液中IL-12、IL-17的表达及临床研究

摘要
目的研究广泛型侵袭性牙周炎(GAg P)患者龈沟液(GCF)中IL-12、IL-17的表达及临床指标的关系。方法选择2010年12月-2013年12月海南省海口市第四人民医院口腔科收治的28例GAg P患者为观察组,同期16例成人慢性牙周炎患者为对照组,采集所有研究对象的龈沟液测定IL-12,IL-17并记录治疗前后指标的变化。结果观察组治疗前牙龈出血指数(BI)、牙周袋深度(PD)、附着丧失(AL)高于对照组,且治疗后明显改善,其中IL-12的表达明显低于对照组,差异有统计学意义(P<0.05),IL-12浓度在治疗后高于治疗前,但差异无统计学意义(P>0.05),治疗后与对照组相比差异有统计学意义(P<0.05);治疗前IL-17总量高于对照组,治疗后3个月GAg PIL-17较治疗前明显下降。GCF中IL-17质量浓度与BI,PD呈正相关(r=0.291,0.330),同AL无明显相关性;IL-12的质量浓度与PD呈显著负相关(r=-0.239,P<0.05),与BI和AL无明显相关性。结论龈沟液中IL-12,IL-17浓度与广泛型侵袭性牙周炎破坏及炎症有关。
        【Objective】To study the expression of IL-12 and IL17 in the gingival crevicular fluid(GCF) of patients with generalized aggressive periodontitis(GAg P) and its correlation with clinical indicators.【Methods】A total of 28 GAg P patients who came to the periodontal department of our hospital from December 2010 to December 2013 were enrolled into observation group and 16 adult patients with chronic periodontitis in the same period were enrolled into control group. The gingival crevicular fluid of all the subjects was collected for the detection of IL-12 and IL-17, and the changes of clinical indicators before and after treatment were also recorded. 【Results】Before the treatment, there were higher bleeding index(BI), deeper probing depth(PD) and greater attachment loss(AL) in the GAg P patients than in the patients of the control group, significant improvements were observed after treatment. The mass concentration of IL-12 in the GCF of the GAg P group was significantly lower than that of the control group,and the difference was statistically significant(P < 0.05). The mass concentration of IL-12 at the sites of periodontitis was higher after treatment than before treatment, but the difference was not statistically significant(P > 0.05). The amount of IL-17 was higher in the GAg P group than in the control group before treatment, which significantly decreased in the GAg P group 3 months after treatment. The mass concentration of IL-17 in GCF was in positive correlation with BI and PD(r = 0.291, 0.330), but not in significant correlation with AL. The mass concentration of IL-12 in GCF was in significantly negative correlation with PD(r =-0.239, P < 0.05), but not in significant correlation with BI or AL.【Conclusions】The concentrations of IL-12 and IL-17 in GCF may be correlated with the destruction and inflammation of GAg P.
