2017、2018年德州市艾滋病患者抗病毒治疗HIV病毒载量抑制效果分析

目的了解德州市艾滋病患者抗病毒治疗HIV病毒载量抑制效果,分析影响因素,为制定抗病毒治疗策略提供科学依据。方法采用回顾性调查,分析2017、2018年艾滋病患者抗病毒治疗末次HIV病毒载量检测数据。结果 347例艾滋病患者接受HAART 12.23～148.47个月,男性312例(89.91%),女性35例(10.09%),男∶女=8.91∶1;年龄为18～71(34.94±10.21)岁;小学及以下文化占8.07%(28/347);婚姻状况以已婚或同居为主(46.40%、161/347);同性性传播为主要传播途径(68.01%、236/347);30.84%(107/347)的艾滋病患者基线CD4细胞计数≤200个/mm~3。285例经过至少1年的抗病毒治疗后体内病毒得到完全抑制,病毒完全抑制率为82.13%。病毒抑制效果的影响因素包括治疗时长(OR=4.49、95%CI=2.31～8.71)、基线CD4细胞水平(OR=2.16、95%CI=1.16～4.03)及服药依从性(OR=4.95、95%CI=2.25～10.86)(P<0.01、<0.05)。结论抗病毒治疗可以有效抑制HIV病毒复制。
Objective To understand the inhibition effect of HIV viral load of antiviral therapy among AIDS patients in Dezhou city,and analyze the influencing factors,so as to provide scientific evidence for developing antiviral treatment strategies. Methods A retrospective survey was conducted to analyze the last HIV viral load data of antiviral therapy for AIDS patients during 2017-2018. Results A total of 347 AIDS patients accepted HAART for 12.23-148.47 months,including 312 males(89.91%)and 35 females(10.09%),the sex ratio was 8.91:1;the age ranged from 18 to 71 years,with the average age of(34.94±10.21)years;people whose education level was the primary school and lower accounted for 8.07%(28/347);the marital status of most cases were married or cohabitation(46.40%,161/347);homosexual transmission was the main route of transmission(68.01%,236/347);CD4 cell count in 30.84%(107/347)of AIDS patients was ≤200/mm~3.There were 285 patients were completely inhibited by antiviral therapy after at least 1 year of antiviral therapy,and the complete inhibition rate of the virus was 82.13%.The factors affecting the viral inhibition include treatment duration(OR=4.49,95%CI=2.31-8.71),baseline CD4(OR=2.16,95%CI=1.16-4.03)and medication compliance(OR=4.95,95%CI=2.25-10.86)(P<0.01、<0.05). Conclusion Antiviral therapy can effectively inhibit HIV virus replication.
