高级迭代重建算法在双低主动脉CTA中的应用研究

摘要
目的探讨双源CT迭代重建算法在双低主动脉CTA中的应用潜能。方法采用随机分组法将108例临床要求行全主动脉CTA扫描的患者分为实验组(A)和对照组(B),A组(n=52):采用80kV,造影剂用量为0.7 mL/kg, B组(n=56):采用100 kV,造影剂用量为1.0 mL/kg,余扫描条件相同。两组图像均应用迭代重建(SAFIRE)与滤波反投影重建(FBP)两两比较,并对各组图像质量进行主观和客观评价:由两名高年资医师分别测量主动脉四个不同层面感兴趣区(ROI)的CT值、噪声(SD),对比噪声比(CNR)并对图像质量进行主观评分。结果 A组SAFIRE重建图像CT值、噪声、CNR优于B组FBP重建差异有统计学意义(P<0.05);而主观评分高于B组FBP重建差异无统计学意义(P>0.05);B组SAFIRE重建图像CT值、噪声、CNR优于A组SAFIRE重建差异有统计学意义(P<0.05),而主观评分高于A组SAFIRE重建差异无统计学意义(P>0.05);B组SAFIRE重建主观评分略高于B组FBP重建差异无统计学意义(P>0.05)。A组DLP(1.81±0.64) mGy.cm较B组DLP(5.48±0.23) mGy.cm辐射剂量减少67%,A组造影剂用量(48.54±12.04) mL较B组用量(67.02±13.47) mL减少28%。结论低电压联合迭代重建算法能够减少图像噪声,提高图像质量,降低辐射剂量、减少造影剂用量。
