下颌第三磨牙拔除后行引导性骨组织再生术对下颌第二磨牙的影响

摘要
目的:比较拔除近中或水平阻生的下颌第三磨牙,拔牙窝行引导性骨组织再生术(guided bone regeneration,GBR),下颌第二磨牙远中牙周情况的变化。方法:收集近中或水平阻生的下颌第三磨牙患者32例,共计64颗下颌第二磨牙,分为实验组(GBR)和对照组(只拔除下颌第三磨牙),于术前、术后3个月、术后6个月和术后1年分别检查记录下颌第二磨牙远中探诊深度、牙槽骨高度和松动度的变化并对比。结果:两组探诊深度在术前无统计学意义(P=0.963),术后3个月、术后6个月和术后1年有差异,且均具有统计学意义(P=0.047,P=0.035,P=0.008)。两组下颌第二磨牙的松动度在术前和术后3个月无统计学意义(P=0.150,P=0.086),术后6个月和术后1年,均有统计学意义(P=0.043,P=0.033)。两组下颌第二磨牙的远中牙槽骨高度在术前无统计学意义(P=0.851),术后3个月、6个月和术后1年,均有统计学意义(P=0.047,P=0.015,P=0.007)。结论:下颌第三磨牙拔除后行GBR,可在术后使下颌第二磨牙远中牙周情况好转。
        Objective: Removing the mandibular third molar in mesioangular impaction and horizaontal impaction,the extraction socket was guided bone regeneration( GBR) or not,the changes of periodontal condition in the mandibular second molar. Methods: 32 patients were collected,a total of 64 mandibular second molars the trial were divided into experimental group( GBR) and control group randomy. The probing depth,the height of alveolar bone and mobility degree were collected 3 months preoperatively,and in 3 and 6 months and 1 year postoperatively,which were compared between the two groups. Results: There was no statistical difference in the probing depth between the two groups before the operation( P = 0. 963),but there were significant differences at 3 and 6 months and 1 year postoperatively( P = 0. 047,P = 0. 035,P = 0. 008). There was no statistical diference in the mobility degree between the two groups( P = 0. 150,P = 0. 086) before and after 3 months after surgery,and there was a statistical difference at 6 months and 1 year postoperatively( P = 0. 043,P = 0. 033). There was no statistical diference in the height of alveolar bone between the two groups before the surgery( P = 0. 851),and there were statistical differences at 3,6 months and 1 year postoperatively( P = 0. 047,P = 0. 015,P =0. 007). Conclusion: The extraction sockets which is removing mandibular third molar with guided bone regeneration,it can improve the periodontal condition of the mandibular second molar.
