中药熏蒸联合针刺治疗颈肩腰腿痛随机平行对照研究

摘要
[目的]观察中药熏蒸联合针刺治疗颈肩腰腿痛疗效。[方法]使用随机平行对照方法,将60例门诊患者按就诊顺序号随机分为两组。对照组30例,颈肩痛:大椎,叩刺,少量出血后停止,每3天1次;腰腿痛:大肠俞、丘墟及环跳等,捻转提插泻法,1次/d。治疗组30例中药熏蒸方(大血藤10g,苏木8g,秦艽20g,防风、桑枝、桂枝、透骨草、三棱、伸筋草、丹参、羌活、络石藤各10g,甘草片6g,威灵仙10g),药材入纱布袋放熏蒸治疗仪,加陈醋和黄酒,疼痛部位熏蒸,1次/d,30min/次;针刺治疗同对照组。连续治疗10d为1疗程。观测临床表现、症状评分、不良反应。治疗1疗程(10d),判定疗效。[结果]治疗组痊愈15例,显效8例,有效5例,无效2例,总有效率93.33%;对照组痊愈5例,显效6例,有效10例,无效9例,总有效率90.00%;治疗组疗效优于对照组(P<0.05)。疼痛评分、功能受限评分、肿胀评分两组均有降低(P<0.01),治疗组降低多于对照组(P<0.01)。[结论]中药熏蒸联合针刺治疗颈肩腰腿痛,疗效满意,无严重不良反应,值得推广。
[Objective] To observe the curative effect of traditional Chinese medicine fumigation combined with acupuncture on neck, shoulder, waist and leg pain. [Method] Sixty outpatients were randomly divided into two groups according to the order of visiting. In the control group, 30 cases had neck and shoulder pain:big vertebra, tapping, stopping after a small amount of bleeding, once every 3 days; low back and leg pain:large intestine shu, hilly ruins and jumping, twisting, lifting, inserting and reducing method, once a day. In the treatment group, 30 Chinese herbal fumigation prescriptions(Daxueteng 10 g, Sumu 8 g, Qinjiao 20 g, Fangfng,Sangzhi, Tougucao, Sanleng, Shenjincao, Danshen, Qianghuo, Luoshiteng 10 g, Gancao 6 g, Weilingxian 10 g)were put into gauze bags and fumigated with vinegar and yellow wine, fumigation of pain parts once a day, 30 minutes per time; the same control group was treated with acupuncture. Continuous treatment for 10 days is a course of treatment. Clinical manifestations, symptom scores and adverse reactions were observed. One course of treatment(10 days) was used to determine the curative effect. [Result] In the treatment group, 15 cases were cured, 8 cases were markedly effective, 5 cases were effective and 2 cases were ineffective, the total effective rate was 93.33%. In the control group, 5 cases were cured, 6 cases were markedly effective, 10 cases were effective and 9 cases were ineffective, the total effective rate was 90.00%. The curative effect of the treatment group was better than that of the control group(P<0.05). Pain score, functional limitation score and swelling score were decreased in both groups(P<0.01), and the decrease in treatment group was more than that in control group(P<0.01). [Conclusion] Traditional Chinese medicine fumigation combined with acupuncture for neck, shoulder, waist and leg pain has satisfactory curative effect and no serious adverse reactions, which is worthy of promotion.
