摘要
目的探讨针刺治疗炎性痛的选穴规律,为临床治疗选穴提供参考。方法检索近5年中国期刊全文数据库(CNKI)、万方数据知识服务平台(WF)、维普数据库(VIP)收录的针刺治疗炎性痛相关临床研究文献,录入Excel 2016及"中医传承辅助平台(V2.5)"对选用的腧穴、经络及常见原因等进行统计分析。结果共纳入文献110篇,涉及119个穴位,总频次576次。总体选穴应用频次由高到低为足三里、合谷、三阴交等。应用频次最多的经脉为足太阳膀胱经。炎性痛的常见病因有腰椎间盘突出症、膝骨性关节炎、手术损伤等。常见病依照病变部位选穴包括,腰椎间盘突出症引起的炎性痛选穴应用频次由高到低为委中、环跳、夹脊穴等;膝骨性关节炎引起的炎性痛选穴应用频次由高到低为足三里、犊鼻、内膝眼等;手术损伤引起的炎性痛选穴应用频次由高到低为合谷、三阴交、足三里等。结论针刺治疗炎性痛在临床应用广泛,且疗效显著。选穴多取阳经穴位,在经络论治和辨证施治的理论上注重远近配穴及特定穴的应用。
        Objective: To explore the rules of acupoints for acupuncture in treatment of inflammatory pain,and to provide reference for clinical treatment of acupoints. Methods: The clinical research articles of acupuncture on inflammatory pain were collected from CNKI,Wanfang Data Knowledge Service Platform(WF) and VIP database in the recent five years,and entered in the Excel 2016 and the TCM Inheritance Platform software(Version 2.5)to analyze the selected acupoints,meridians and common causes. Results: 110 articles were included,involving119 acupoints,with a total frequency of 576. The overall frequency of selection points from high to low was Zusanli(ST36),Hegu(LI4),Sanyinjiao(SP6),etc.. The most frequently used Bladder Meridian was Foot-Taiyang meridians. The common causes of inflammatory pain included protrusion of lumbar intervertebral disc,osteoarthritis of the knee,and surgical injury. Selection of acupoints for common diseases according to the location of lesions: the frequency of inflammatory pain selection caused by protrusion of lumbar intervertebral disc from high to low was Weizhong(BL 40),Huantiao(GB 30),Jiaji(Ex-BZ) etc..;the frequency of inflammatory pain selection caused by osteoarthritis of the knee from high to low was Zusanli(ST 36),Dubi(ST 35),Neixiyan(Ex-LE5)etc..;Surgical injury induced inflammatory pain acupoint application of frequency from high to low was Hegu(LI4)and Sanyinjiao(SP 6),Zusanli(ST 36) etc,. Conclusion: Acupuncture is widely used in the treatment of inflammatory pain,and the effect is remarkable. Acupoints of Yangjing meridian are often selected,and the application of distant and near acupoints and specific acupoints is emphasized in the theory of meridian treatment and syndrome differentiation.
