温针灸对恢复期脑梗死患者脑血流灌注情况及血清ICAM-1、IGF-1的影响

摘要
目的观察温针灸对恢复期脑梗死患者脑血流CT灌注情况及细胞间黏附分子-1(ICAM-1)、胰岛素样生长因子-1(IGF-1)水平的影响。方法将80例恢复期脑梗死患者随机分为治疗组和对照组,每组40例。对照组采用常规药物治疗及康复训练,治疗组在对照组基础上采用温针灸治疗。治疗8周后,观察两组治疗前后美国国立卫生研究院卒中量表(NIHSS)评分、Fugl-Meyer运动功能评分法评分、脑CT灌注成像各项指标[局部脑血流量(rCBF)、局部脑血容积(rCBV)、平均通过时间(MTT)]及血清ICAM-1、IGF-1水平的变化情况。结果两组治疗后NIHSS评分、FMA评分、脑CT灌注成像各项指标及血清ICAM-1、IGF-1水平与同组治疗前比较,差异均具有统计学意义(P<0.01,P<0.05)。治疗组治疗后NIHSS评分、FMA评分、脑CT灌注成像各项指标及血清ICAM-1、IGF-1水平与对照组比较,差异均具有统计学意义(P<0.01,P<0.05)。结论温针灸可降低恢复期脑梗死患者血清ICAM-1水平,提高IGF-1水平,有效改善脑血流灌注水平,从而有助于神经功能恢复。
        Objective To investigate the effect of warm needling moxibustion on cerebral blood perfusion by CT scan and the levels of serum intercellular adhesion molecule-1 (ICAM-1) and insulin-like growth factor 1 (IGF-1) in patients with cerebral infarction in the convalescent stage. Method Eighty patients with cerebral infarction in the convalescent stage were randomized into treatment and control groups, with 40 cases in each group. The control group was intervened by conventional medication plus rehabilitation training, while the treatment group was additionally given warm needling moxibustion. Before and after 8-week treatment, the national institute of health stroke scale (NIHSS) score, Fugl-Meyer assessment (FMA) of motor function score, CT indicators of cerebral blood perfusion [regional cerebral blood flow (rCBF), region cerebral blood volume (rCBV) and mean transit time (MTT)] and the levels of serum ICAM-1 and IGF-1 in the two groups were observed. Result NIHSS score, FMA score, CT indicators of cerebral blood perfusion and the levels of serum ICAM-1 and IGF-1 all showed a significant change after the treatment in the two groups (P<0.01, P<0.05). After the treatment, NIHSS score, FMA score, CT indicators of cerebral blood perfusion and the levels of serum ICAM-1 and IGF-1 in the treatment group were significantly different from those in the control group (P<0.01, P<0.05). Conclusion Warm needling moxibustion can down-regulate serum ICAM-1 level and up-regulate serum IGF-1 level in patients with cerebral infarction in the convalescent stage, and it can also effectively improve the level of cerebral blood perfusion in the patients, which helps to recover the nerve function.
