蔡宛如教授从“脏腑之气”分治慢性阻塞性肺疾病

摘要
[目的]探讨蔡宛如教授从脏腑之气出发治疗慢性阻塞性肺疾病的经验。[方法]通过门诊跟师学习,整理蔡宛如教授相关医案,从脏腑气机理论、处方用药等方面分析总结蔡宛如教授治疗慢性阻塞性肺疾病的临证经验,并附验案两例。[结果]蔡宛如教授认为,慢性阻塞性肺疾病的"咳、喘"是由于脏腑气机的升降出入异常引起,而"痰"随气病而生。本病多累及肺脾肾等脏。针对慢阻肺患者的"咳、痰、喘",需从整体出发,调畅气机,使"脏腑之气"升降恢复正常。蔡宛如教授运用肝肺同治、健脾和胃、扶正益气等法治疗慢性阻塞性肺疾病,均取得良好的临床疗效。所附医案一属肺虚痰热证,一属气阴两虚证,运用补气、降气等法调节脏腑之气,均获验效。[结论]蔡宛如教授从"脏腑之气"分治慢性阻塞性肺疾病,疗效颇著,值得借鉴。
[Objective] To explore Professor CAI Wanru's experience in treating chronic obstructive pulmonary disease from functional activities of Qi of Zang-fu. [Method] Through the follow-up study of the outpatient clinic, collect the relevant medical records of Professor CAI Wanru, analyze and summarize Professor CAI Wanru's clinical experience in treating chronic obstructive pulmonary disease from Qi of Zang-fu and prescription medication,and attach two medical records for example. [Results] Professor CAI Wanru believes that the "cough, asthma" of chronic obstructive pulmonary disease is caused by abnormal movement of Qi of Zang-fu, and "phlegm" is born with Qi disease. This disease involves the lungs, spleen and kidneys and other organs. For the symptoms of "cough, phlegm and asthma" in patients with chronic obstructive pulmonary disease, it is necessary to start from the whole,adjust the Qi movement, and let the"Qi of Zang-fu"return to normal. Professor CAI Wanru has achieved good clinical results in the treatment of chronic obstructive pulmonary disease with the methods of liver and lung treatment, reinforcing spleen and stomach, and strengthening and invigorating genuine Qi.There are two medical records, one is lung deficiency and phlegm-heat, the other is Qi and Yin deficiency, it's effective to adjust the Qi of Zang-fu by reinforcing Qi and lowering Qi.[Conclusion] Professor CAI Wanru's treatment of chronic obstructive pulmonary disease from the "Qi of Zang-fu" is quite effective and worth learning.
