摘要
目的研究~(18)F-FDG(氟代脱氧葡萄糖)PETCT(正电子发射型计算机断层显像)的最大标准摄取值(SUVmax)与PSA(前列腺特异抗原)在前列腺癌(PCa)诊治中的相关性。方法选取我院诊治的37例经病理学诊断确诊为前列腺癌的患者作为观察组,收集28例良性前列腺增生患者作为对照组。两组患者分别行PSA筛查以及~(18)F-FDG PETCT检查。以SUVmax>2.5、自由PSA/总PSA(f/t)<0.16作为诊断前列腺癌标准。研究人员对两组患者经~(18)F-FDG PETCT生成的图像中放射性分布阳性病灶进行感兴趣区测量,计算SUVmax。统计分析SUVmax与PSA相关性。结果观察组37例t PSA 4-10ng/m L者28例,t PSA>10ng/m L者9例;对照组28例患者中,t PSA 4-10ng/m L者17例,t PSA<4ng/m L者11例。观察组f/t显著低于对照组;观察组SUAmax显著高于对照组。经相关性分析,SUVmax与f PSA正相关(t=0.877,P<0.05);SUVmax与f/t负相关(t=0.988,P<0.05)。结论SUVmax与f PSA正相关,SUVmax与f/t负相关,对于处于诊断灰区的前列腺疾病患者,SUVmax结合f/t可显著提高诊断前列腺癌的准确率。
        Objective To study the collelation between the ~(18)F-FDG(FDG) PETCT(positron emission tomography) maximum standardized uptake value(SUVmax) and PSA(prostate specific antigen) in the diagnosis and treatment of prostate cancer. MethodsA total of 37 cases which diagnosed of prostate cancer were selected as the observation group, and 28 cases of benign prostatic hyperplasia patients as a control group. Two groups of patients underwent PSA screening and ~(18)F-FDG PETCT inspection. In SUVmax> 2.5, free PSA/total PSA(f/t) <0.16 as the standard diagnosis of prostate cancer. Researchers two groups of patients by ~(18)F-FDG PETCT generated image of radioactivity distribution of positive lesions ROI measurement, calculation SUVmax. Statistical analysis SUVmax correlation with PSA. Results 37 cases t PSA 4-10 ng/m L in 28 cases, t PSA> 10 ng/m L in 9 cases, control group of 28 patients, t PSA 4-10 ng/m L in 17 cases, t PSA <4 ng/m L in 11 cases. Observer Group f/t significantly lower than the control group. SUAmax observation group was significantly higher. After correlation analysis, SUVmax and f PSA positive correlation(t=0.877, P<0.05). SUVmax with f/t negative correlation(t=0.988, P <0.05). Conclusion SUVmax positively correlated with f PSA, SUVmax with f/t negative correlation for patients with prostate disease in diagnostic gray zone, SUVmax combined with f/t can significantly improve the accuracy of diagnosis of prostate cancer.
