肛周超声检查在肛周脓肿术前诊断的价值

摘要
为探讨肛周超声检查在肛周脓肿术前诊断的价值,2017年2月至2018年2月高密市夏庄中心卫生院对术前超声检查定位脓腔及内口明确的60例化脓期肛周脓肿患者行肛周脓肿一期根治术,现将术前超声诊断与术中诊断结果进行对比分析,并回顾手术治疗效果。结果显示,与术中诊断比较,术前肛周超声检查肛周脓肿脓腔符合率为100%,内口位置符合率为95.0%。痊愈57例,后遗肛瘘3例,治愈率95.0%。结果表明,肛周超声检查可以确定肛周脓肿的位置、大小及其与周围组织的关系,并准确定位内口,在肛周脓肿的定位方面具有重要的作用;而且肛周超声操作简单,可提高肛周脓肿一期手术成功率,避免二次手术。
To investigate the preoperative diagnostic value of perianal ultrasound inspection(UI) for perianal abscess(PA),60 cases of PA,with definite location of pus cavity and internal opening by preoperative UI in author's clinic from Feb. 2017 to Feb. 2018 underwent primary radical surgery during their suppuration, then their preoperative UI results were analysis and compared with intraoperative diagnostic outcome,and curative effect was reviewed retrospectively. As results,both diagnosis accordance rate were 100% in pus cavity,95.0% in internal opening situs,in the 60 cases 57 were cured,3 suffered from sequela fistula in ano,thus curative rate was 95.0%.Results show that perianal UI can determine PA location,size and its relation with surrounding tissue of pus cavity accurately locate internal opening situs, which plays an important role in positioning of PA and its manipulation is simple,can enhance successful rate of PA primary surgery and avoid secondary surgery.
