人工髋关节置换术治疗高龄不稳定股骨粗隆间骨折的疗效分析

摘要
目的探讨人工髋关节置换术治疗高龄不稳定型股骨粗隆间骨折患者的疗效。方法选取我院2015年1月至2017年1月收治的48例高龄不稳定型股骨粗隆间骨折患者,随机分为两组各24例,观察组采用人工髋关节置换术治疗,对照组采用股骨近端防旋髓内钉内固定术治疗。比较两组的治疗效果。结果观察组的手术时间、术中出血量均显著大于对照组,术后下床时间显著短于对照组(P <0.05)。术后1个月、 3个月,观察组的Harris评分显著高于对照组(P <0.05);术后6个月,两组的Harris评分无统计学差异(P>0.05)。结论人工髋关节置换术是治疗高龄不稳定型股骨粗隆间骨折患者的一种有效方法 。
Objective To explore the efficacy of artificial hip joint replacement for the treatment of elderly patients with unstable intertrochanteric fracture. Methods 48 cases of elderly patients with unstable intertrochanteric fracture admitted to our hospital from January2015 to January 2017 were selected and randomly divided into two groups, with 24 cases in each group. The observation group was treated with artificial hip joint replacement, while the control group was treated with internal fixation with proximal femoral nail. The curative effects of two groups were compared. Results The operation time and intraoperative blood loss of the observation group were significantly higher than those of the control group, and the postoperative out-of-bed time was significantly shorter than that of the control group(P <0.05).At 1 month and 3 months after surgery, the Harris score of the observation group was significantly higher than that of the control group(P<0.05); At 6 months after surgery, no significant difference was found between two groups in Harris score(P >0.05). Conclusions Artificial hip joint replacement is an effective method for the treatment of elderly patients with unstable intertrochanteric fracture.
