2013～2017年大连市甘井子区5岁以下儿童死亡监测分析

摘要
目的了解大连市甘井子区5岁以下儿童死亡状况,为儿童死亡防控提供科学依据。方法采用5岁以下儿童死亡监测网收集2013～2017年大连市甘井子区5岁以下儿童死亡资料,对监测报告结果进行分析。结果 2013～2017年大连市甘井子区活产数47 450人,5岁以下儿童死亡172名。新生儿死亡率、婴儿死亡率及5岁以下儿童死亡率分别为2.02‰、2.62‰和3.62‰。占5岁以下儿童死亡的构成比,0～7 d的占39.53%,新生儿死亡占55.81%,婴儿死亡占76.74%。5岁以下儿童死亡病例未治疗的高达27.91%,死亡地点在医院的仅占44.77%(在家中的占40.12%)(P>0.05)。1岁以下儿童前5位死因占比依次是其他先天异常(26.47%)、其他新生儿疾病(24.51%)、早产或低出生体重(23.53%)、出生窒息(12.75%)、先天性心脏病(4.90%);5岁以下儿童的情况与之相似。结论新生儿死亡率、婴儿死亡率及5岁以下儿童死亡率都下降。
Objective To understand the mortality status of children under 5 years old in Ganjingzi district,Dalian city during 2013-2017,so as to provide scientific basis for prevention and control of children death. Methods The death data of children under 5 years old in Ganjingzi district,Dalian city was collected from death monitoring network of children under 5 years old during 2013-2017. Results A total of 172 deaths of children under 5 years old were registered among 47 450 live births in Ganjingzi district,Dalian city during 2013-2017.The neonatal mortality rate(NMR),infant mortality rate(IMR)and mortality under age 5(U5 MR)was 2.02‰,2.62‰ and 3.62‰ respectively.Among the death cases under 5 years old,death in the first week,month and year accounted for 39.53%,55.81% and 76.74% respectively;and the number of untreated cases was 27.91%,only 44.77% cases died in the hospital,and 40.12% died at home(P>0.05).For the children death under 12 months old,the top 5 death causes were other congenital anomaly(OCA),other neonatal diseases(OND),preterm delivery or low birth weight,birth asphyxia and congenital heart disease(CHD),accounting for 26.47%,24.51%,23.53%,12.75% and 4.90% respectively,with the similar sequence to the death causes of children under 5 years old. Conclusion It shows decrease trend in NMR,IMR and U5 MR.
