甲磺酸伊马替尼致扁平苔藓样药疹1例及文献复习

摘要
患者女,71岁。全身出现红斑、丘疹伴瘙痒2个月。患者2个月前口服"甲磺酸伊马替尼片"等药物,服药5天后躯干、四肢近端出现红斑及丘疹,逐渐泛发全身,瘙痒剧烈,既往2年前曾口服"甲磺酸伊马替尼片"出现类似皮疹,经治疗痊愈。左下肢红斑组织病理示:表皮角化过度伴角化不全,基底层细胞液化变性,真皮带状致密炎症浸润,主要为淋巴细胞,少量嗜酸性粒细胞。诊断:扁平苔藓样药疹。给予抗过敏、抗炎、止痒等治疗后好转。
A 71-year-old female patient presented with erythemas and papules all over the body for two months,which due to the use of imatinib mesylate. Five days later she developed erythemas and plaques on the trunk and limbs,and the skin lesion progressed to the whole body gradually. She had presented similar lesion because of using imatinib mesylate two years ago. Skin biopsy was performed on his left lower leg. Specimen showed epidermal hyperkeratosis with parakeratosis,liquefaction degeneration of the basal cell layer,lichenoid lymphocytic infiltration and fewer numerous eosinophils infiltration on the upper dermis and deep dermis. It was diagnosed as lichenoid drug eruption according clinical manifestations and auxiliary examination. Anti-allergy,anti-inflammatory and anti-itching were given as the treatment.
