乌鲁木齐市某医院脑卒中后吞咽障碍患者照顾者负担水平及其影响因素

摘要
目的:探讨脑卒中后吞咽障碍患者照顾者负担水平及影响因素。方法:选取2014年1月-2016年12月乌鲁木齐市某医院康复医学科收治的84例脑卒中后吞咽障碍患者的照顾者作为研究对象,采用一般资料调查问卷、Zarit负担量表、简易应对方式问卷(SCSQ)对所有照顾者进行评估,并确定影响脑卒中后吞咽障碍患者照顾者负担水平的主要因素。结果:脑卒中后吞咽障碍患者照顾者负担处于轻、中度水平;SCSQ积极应对、消极应对得分均明显低于常模(P<0.05);应用统计学中的多元线性分析,照顾者消极应对方式、照顾时间2-3年、年龄为55-70岁、中度/重度吞咽障碍与照顾者负担水平呈正相关。结论:脑卒中后吞咽障碍患者照顾者的负担处于轻/中度水平,其负担与应对方式密切相关,护理人员在对脑卒中后吞咽障碍患者进行护理的同时,需重视其照顾者心理问题,引导照顾者采取正确的应对方式,减轻照顾负担。
Objective: To investigate the burden level and its influencing factors of the caregiver in patients with dysphagia after stroke. Methods: A total of 84 caregivers for patients with post-stroke dysphagia admitted to rehabilitation medicine department of a hospital in Urumqi from January 2014 to December 2016 were selected and investigated by the general information questionnaire, the Zarit caregiver burden interview, and the simple coping style questionnaire(SCSQ). The main factors affecting the burden level of caregivers were studied. Results: The burden of caregivers of patients with dysphagia after stroke was light and moderate. The scores of SCSQ positive coping and negative coping of caregivers of patients with dysphagia after stroke were significantly lower than the norm(P<0.05). Care duration of 2-3 years, age of 55-70 years, moderate/severe dysphagia were positively correlated with the burden level of caregivers. Conclusion: The burden on caregivers of patients with dysphagia after stroke is at a light/moderate level and it is closely related to their coping style. The nursing staff should pay psychological attention to their caregivers and guide them, in order to ease their burden level..
