摘要
目的:探讨原发性高血压患者乙醛脱氢酶2(aldehyde dehydrogenase 2, ALDH2)基因多态性与血清高敏C反应蛋白水平的关系。方法:将246名中国南方汉族人(无亲缘关系)分成2组,高血压组123例,对照组123名。测定2组ALDH2的基因分型和血清高敏C反应蛋白水平,比较2组受试者ALDH2不同基因型的血清高敏C反应蛋白水平的差异。结果:246例中国南方汉族人的ALDH2基因型有3种,即ALDH2野生纯合子型(ALDH2*1/*1)、ALDH2突变杂合型(ALDH2*1/*2)和ALDH2突变纯合子型(ALDH2*2/*2),在高血压组中分别为69例(56.1%)、43例(35.0%)和11例(8.9%),在对照组中分别为72例(58.5%)、47例(38.2%)和4例(3.3%),这3种基因型在所检测人群中的分布符合Hardy-Weinberg定律(P=0.637)。对照组与高血压组之间ALDH2基因型分布差异无统计学意义(P=0.699)。高血压组血清高敏C反应蛋白水平高于对照组[0.92±0.87比0.70±0.67(对数值),P<0.05]。高血压组中ALDH2突变型(突变杂合型+突变纯合子型)血清高敏C反应蛋白水平高于野生型[1.10±1.03比0.77±0.69(对数值),P<0.05]。经多元线性回归分析,校正年龄、性别、2型糖尿病病史、体质量指数、血清总胆固醇、低密度脂蛋白胆固醇、晨起基础收缩压和晨起基础舒张压后,ALDH2基因多态性是血清高敏C反应蛋白水平的影响因素(P=0.039)。结论:高血压患者ALDH2基因多态性可能对血清高敏C反应蛋白水平存在一定影响。
        Objective To investigate the association between gene polymorphism of aldehyde dehydrogenase 2(ALDH2) and serum high-sensitivity C-reactive protein level among essential hypertension patients. Methods A total of246 unrelated Southern Chinese Han people were divided into hypertension group(n =123) and control group(n =123).ALDH2 genotype and serum high-sensitivity C-reactive protein level were detected. The serum high-sensitivity C-reactive protein level was compared between the two groups with different genotype. Results There were three genotypes of ALDH2, including wild-type ALDH2(ALDH2*1/*1), mutant heterozygous type ALDH2(ALDH2*1/*2) and mutant homozygous type ALDH2(ALDH2*2/*2). The distribution of the three ALDH2 genotypes were in accordance with Hardy-Weinberg equilibrium(P =0.637). In the hypertension group, the number and proportion of three genotypes were 69 and56.1%, 43 and 35.0%, 11 and 8.9%, respectively. Meanwhile in the control group the number and proportion were 72 and58.5%, 47 and 38.2%, 4 and 3.3%, respectively. No significant difference was found in genotype proportion of ALDH2 between the hypertension group and control group(P=0.699). The serum high-sensitivity C-reactive protein level of hypertension group was significantly increased when compared with the control group(0.92±0.87 LN vs 0.70±0.67 LN, P<0.05). In hypertension group, the serum high-sensitivity C-reactive protein level of mutant type ALDH2(ALDH2*1/*2 and ALDH2*2/*2) was increased when compared with the wild-type ALDH2(1.10±1.03 LN vs 0.77±0.69 LN, P<0.05). Multiple linear regression analysis showed that after the adjustment of age, gender, history of type 2 diabetes, body mass index,serum levels of cholesterol, low density lipoprotein-cholesterol, basic systolic blood pressure and diastolic blood pressure,gene polymorphism of ALDH2 was the influencing factor of serum high-sensitivity C-reactive protein level in hypertension group(P=0.039). Conclusions Serum level of high-sensitivity C-reactive protein may be influenced by polymorphism of ALDH2 in essential hypertension patients.
