摘要
目的了解钦州市近年新发现报告HIV/AIDS病例的病毒基因亚型及传播簇的特征。方法按方便抽样方法选取2012—2014年确证实验阳性患者剩余的血浆样本进行HIV pol区基因片段扩增和序列测定,对获得的核酸序列进行系统进化树构建并鉴定毒株的基因亚型,分析基因亚型分布及传播簇特征。结果 2012—2014年钦州市新发现报告1 558例HIV/AIDS患者,纳入pol区基因扩增167例,扩增阳性147例,有139例获得pol区基因序列。亚型分析发现CRF01_AE、 CRF07_BC、 CRF08_BC等3种基因亚型和1种独特重组型(URF),分别占基因序列分析例数的72.66%、 3.60%、 23.02%和0.72%。在经异性传播途径传播的120例病例中, CRF01_AE所占的比例最高(76.67%),其次是CRF08_BC (18.33%)和CRF07_BC (3.60%)。在经注射吸毒途径传播的13例病例中,仅发现CRF01_AE(46.15%)和CRF08_BC (53.85%)两种基因亚型。101例主要流行CRF01_AE亚型毒株聚集成5个传播簇Cluster 1、2、 3、 4、 7,分别占23.76%、 68.32%、 5.94%、 0.99%、和0.99%,以第2传播簇为主。结论钦州市近年流行的HIV-1以CRF01_AE为优势毒株,经异性途径感染的毒株以CRF01_AE为主要毒株且以第2传播簇为主,而在经注射吸毒感染的毒株中CRF08_BC占一半以上,提示钦州市HIV传播仍以本地流行的毒株为主。
        Objective To understand the characteristics of HIV subtypes and their transmission clusters among the newly reported HIV/AIDS cases in Qinzhou from 2012-2014.Methods The remaining plasma samples of the patients with positive HIV test from 2012 to 2014 were selected based on the convenient sampling method and the pol gene fragment of HIV were amplified and sequenced.The subtypes and their transmission clusters were identified by the phylogenetic tree analyzing,which was constructed with the obtained nucleic acid sequence of the pol gene fragments.Results From 2012 to 2014 there were 1 558 newly reported cases of HIV/AIDS in the city and a total of 167 plasma samples were amplified for the genes of pol region.In the 147 samples positive with the amplification,139 sequences of the pol region gene were obtained.Subtype analysis found three subtypes,CRF01_AE,CRF07_BC,CRF08_BC and a unique recombinant form(URF),accounting for 72.66%,3.60%,23.02% and 0.72%,respectively.Among 120 cases of heterosexual transmission route,the proportion of subtye CRF01_AE was the highest(76.67%),followed by CRF08_BC(18.33%)and CRF07_BC(3.60%).Only two subtypes,CRF01_AE(46.15%) and CRF08_BC(53.85%),were detected among 13 cases transmitted by injecting drug uses. Five transmission clusters,Cluster 1,2,3,4,and 7,were found in 101 cases of subtype CRF01_AE,accounting for 23.76%,68.32%,5.94%,0.99% and 0.99%,respectively.The Cluster 2 was dominant in CRF01_AE strains found in this study.Conclusion The dominant subtye of HIV-1 circulated in Qinzhou in the recent years is CRF01_AE,which is also the dominated subtye found in the cases with the heterosexual transmission route,and the Cluster 2 is the main propagation cluster of the CRF01_AE.CRF08_BC accounts for more than half of the subtyes infected via injecting drug route.These findings suggest that HIV-1 transmission in Qinzhou might be predominantly derived from the local strains.
