3D-TVS诊断子宫粘膜下肌瘤与肌壁间肌瘤的价值探析

摘要
目的探析经阴道三维超声(3D-TVS)诊断子宫粘膜下肌瘤与肌壁间肌瘤的临床价值。方法方便选取该院2017年3月—2018年6月收治的子宫粘膜下肌瘤与肌壁间肌瘤患者共90例,均经病理检查确诊,其中子宫粘膜下肌瘤患者34例,肌壁间肌瘤患者56例。对其3D-TVS、经阴道二维超声(2D-TVS)、经腹部超声检查结果进行比较,探究子宫粘膜下肌瘤、肌壁间肌瘤诊断中3D-TVS检查的应用价值。结果 3D-TVS诊断肌壁间肌瘤的符合率为92.9%,2D-TVS、经腹部超声诊断的符合率分别为78.6%和69.6%,3D-TVS诊断的符合率高于2D-TVS、经腹部超声,对比差异有统计学意义(χ~2=4.666 7、9.904 8,P<0.05);3D-TVS诊断子宫粘膜下肌瘤的符合率为94.1%,2D-TVS、经腹部超声诊断的符合率分别为76.5%和61.8%,3D-TVS诊断的符合率高于2D-TVS、经腹部超声,对比差异有统计学意义(χ~2=4.220 7、10.349 7,P<0.05)。结论 3D-TVS诊断的操作方便、安全性高、检查高效,其以最为可靠的切面对肿块表面及断层进行三维超声扫描,可以获取更为准确、可靠的诊断依据,故采取3D-TVS对子宫粘膜下肌瘤与肌壁间肌瘤进行诊断,具有较高的临床价值。
Objective To analyze the clinical value of transvaginal three-dimensional ultrasound(3D-TVS) in the diagnosis of uterine submucosal fibroids and myometrial fibroids. Methods A total of 90 patients with uterine submucosal fibroids and myometrial fibroids admitted to our hospital from March 2017 to June 2018 were convenient enrolled. All patients were diagnosed by pathological examination. Among them, 34 patients with uterine submucosal fibroids, between the muscle wall56 patients with fibroids. Results The coincidence rate of 3D-TVS diagnosis of myometrial fibroids was 92.9%. The coincidence rate of 2D-TVS and transabdominal ultrasound diagnosis was 78.6% and 69.6%, respectively. The coincidence rate of 3DTVS diagnosis was higher than 2D-TVS, abdominal ultrasound, the difference was statistically significant(χ~2=4.666 7, 9.9048, P<0.05); the coincidence rate of 3 D-TVS diagnosis of uterine submucosal fibroids was 94.1%, and the coincidence rate of 2D-TVS and transabdominal ultrasound diagnosis was 76.5% and 61.8%, 3D-TVS diagnosis was higher than that of 2DTVS and transabdominal ultrasound, and the difference was statistically significant(χ~2=4.220 7, 10.349 7, P<0.05). Conclusion 3D-TVS diagnosis is convenient, safe and effective. It can be used to obtain a more accurate and reliable diagnosis basis by using the most reliable three-dimensional ultrasound scanning on the surface and fault of the tumor. Therefore,3D-TVS is adopted for the diagnosis of uterine submucosal fibroids and myometrial fibroids has high clinical value.
