新诊断2型糖尿病患者血浆SFRP4与PGRN水平变化及其与胰岛素抵抗的相关性研究

摘要
目的:探讨新诊断T2DM患者血浆分泌型卷曲相关蛋白4(SFRP4)与颗粒蛋白前体(PGRN)水平变化与IR的相关性研究。方法:选取2018-01~2018-06佳木斯大学附属第一医院就诊的新诊断2型糖尿病组60例,其中,非肥胖T2DM组30例(BMI<25kg/m~2)和超重肥胖T2DM组30例(BMI≥25 kg/m~2),另选同期健康体检者30例为健康对照组,ELISA检测SPRF4和PGRN水平,同时测身高、体重、腰围、臀围、FPG、HbA1c、TG、TC、HDL-C、LDL-C、Fins等指标,计算BMI、WHR、胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β),分析新诊断T2DM患者SPRF4和PGRN水平与IR的关系。结果:①新诊断T2DM血浆SFRP4、PGRN水平升高。②SFRP4与体重、BMI、FPG、HbA1c、TG、TC、LDL-C、HOMA-IR成显著正相关(P<0.05或P<0.01),与Fins、HDL-C、HOMA-β成显著负相关(P<0.05或P<0.01);PGRN与体重、BMI、WHR、FPG、HbA1c、Fins、TG、HOMA-IR成显著正相关(P<0.05或P<0.01),与HDL-C、HOMA-β成显著负相关(P<0.05或P<0.01)。③多元线性回归结果显示,Fins、HOMA-IR、体重是血清SFRP4水平的独立危险因素;HbA1c、BMI是PGRN血清水平的独立危险因素。结论:2型糖尿病及肥胖患者血浆SFRP4、PGRN水平均明显升高,与T2DM、肥胖及IR相关。
        Objective: To explore the relation between serum Secreted frizzled-related protein 4(SFRP4), Progranulin(PGRN) levels and insulin resistance. Methods: A total of 60 patients with newly diagnosed type 2 diabetes were selected from January 2018 to June 2018 in the first affiliated hospital of Jiamusi University. Among them, there were 30 cases(BMI<25 kg/m~2) in the non-obese T2 DM group and 30 cases(BMI≥25 kg/m~2) in the overweight obesity T2 DM group. 30 healthy subjects were selected as the control group. The Secreted frizzled-related protein 4(SFRP4), Progranulin(PGRN) levels were measured by enzyme linked immune sorbent assay(ELISA), FPG, HbA1 c, TG, TC, HDL-c, LDL-c and Fins were measured simultaneously. BMI, HOMA-IR and HOMA-β were calculated. The relationship between SPRF4 and PGRN levels and IR in newly diagnosed T2 DM patients was analyzed. Results:(1)The plasma concentration of SFRP4 and PGRN in T2 DM was increased.(2) SFRP4 was negatively correlated with weight, BMI, FPG, HbA1 c, TG, TC, LDL-C, HOMA-IR(P<0.05或P<0.01), and positively correlated with Fins, HDL-C, HOMA-β(P<0.05或P<0.01). PGRN was negatively correlated with weight, BMI, WHR, FPG, HbA1 c, Fins, TG, HOMA-IR(P<0.05或P<0.01), and positively correlated with HDL-C, HOMA-β(P<0.05 or P<0.01).(3) Multiple stepwise regression analysis showed that Fins, HOMA-IR and weight were independently associated with SFRP4. HbA1 c and BMI were independently associated with PGRN. Conclusion: Plasma SFRP4 and PGRN levels were significantly increased in type 2 diabetes and obese patients, which were related to T2 DM, obesity and IR.
