皮肤假腺样鳞状细胞癌1例

摘要
患者女,80岁。右颞部肿物2个月余,伴疼痛1个月余。皮损组织病理示:低倍镜下表皮部分缺失,真皮层内可见大量上皮样细胞浸润性生长,呈巢团状、条索状生长,与表皮不相连,间质见黏液样变。高倍镜下可见囊性腔隙,细胞连接松散,腔内可见脱落的棘突松解细胞,胞质丰富嗜酸,异型性明显,可见典型核分裂象。免疫组织化学标记:p63(+),EMA(+),Ki-67热点区域45%(+),CEA(-),HMB45(-)。诊断:(棘层松解性)假腺样鳞状细胞癌。
A 80-year-old female presented with rash on right temporal for 2 month,with pain for 1 month. Histopathological examination showed that the partial absence of epidermis,and a large number of epithelioid cells in the dermis,which in a nested,rod-like growth with mesenchymal changes,not connected to the epidermis. The acantholytic cells could be seen in the cavity. The cell obvious atypia and cytoplasm is eosinophilic.Mitosis could be seen. Immunohistochemistry showed positive staining for P63 EMA,Ki-67,negative for CEA and HMB45. The diagnosis of pseudoglandular squamous cell carcinoma was made.
