胰十二指肠术后拔除PTCD管致肝动脉出血1例

<正>病例男,55岁,因"皮肤巩膜进行性黄染1个月"入院,查体见皮肤、巩膜黄染,腹部专科查体无特殊。入院检查腹部CT提示"胰腺钩突占位",总胆红素340.1 mol/L,Hb 124 g/L,CA 199,20.55 U/ml。入院诊断:胰腺钩突部肿瘤。于入院当日行经皮肝穿刺胆道引流术(PTCD),术后胆汁经PTCD引流约300～600 ml/d。10 d后复查总胆红素降为224.6 mol/L,次日行胰十二指肠切除术,并保留PTCD
