摘要
突发事件的发生会为医院手术室的运作管理带来很大的冲击,如大量急诊手术需求的激增、原定择期手术的推迟、特定医护资源的紧缺等.在常规手术室排程优化问题基础上进一步考虑突发事件特点,同时协调优化择期手术和急诊手术,以各类病人当量等待时间最短、关键资源利用率最高和同质资源利用率均衡为多目标决策,构建优化调度数学模型.设计若干算例验证优化效果,实验结果表明,在保证急诊手术优先的原则下,手术室优化调度提高急诊关键手术资源的利用率,而且均衡各资源的工作时间和工作量,对实际医院在突发事件下的运作调度提供参考.
        Emergency usually can have a great impact on the operation management of operating rooms(OR) in hospital, such as the sudden surge in emergency surgery demands、 the postponement of scheduled elective surgeries、 the shortage of specific healthcare resources etc. Based on the elective surgery scheduling optimization and further considered the characteristics of emergency, an OR scheduling optimization model under emergency is proposed to coordinate the elective surgery and emergency surgery and using, the shortest comprehensive patients waiting time、 the highest critical resource utilization and the balanced resource utilization of homogenous resources as the multi-objectives, a mathematical model for scheduling optimization. Several test cases are designed to verify the optimization result. The results show that the multi-objective under the principle of ensuring emergency surgery priority, OR scheduling optimization can improves the utilization of critical surgical resources, balance the working hours and each resource workload, and provide reference for the actual OR operation management of the hospital under emergency.
