孕早期空腹血糖筛查对妊娠期糖尿病的预测价值

摘要
目的探讨妊娠期糖尿病孕妇孕早期应用空腹血糖筛查的临床诊断价值。方法临床纳入2015年10月—2017年10月期间在该院接受产前检查的300例孕妇作为研究对象,将150名健康孕妇作为对照组,150例疑似妊娠期糖尿病孕妇作为观察组,两组孕妇均进行孕早期空腹血糖筛查,所有孕妇在孕周24～28周时进行75 g葡萄糖耐量试验(OGTT),观察两组孕早期空腹血糖值,评估两组24～28周OGTT时血糖指标。结果对照组孕妇孕早期空腹血糖值较观察组更低(P<0.05);对照组孕妇服用葡萄糖前、服用后1h以及服用后2 h血糖值均低于观察组(P<0.05)。结论妊娠期糖尿病孕妇进行孕早期空腹血糖筛查提升空腹血糖较高,于孕周24～28周时进行75 g OGTT检测时,服用葡萄糖前、服用后1 h以及服用后2 h血糖指标均明显升高。
Objective To study the value of fasting blood glucose screening in the early pregnancy in predicting the gestational diabetes. Methods 300 cases of patients for prenatal examination in our hospital from October 2015 to October 2017 were selected and divided into two groups with 150 cases in each, the control group were healthy pregnant women, while the observation group were pregnancy women suspected with gestational diabetes, both groups were for fasting blood glucose screening in the early pregnancy, and all patients were for 759 OGTT at 24 to 28 weeks, and the early fasting blood glucose values of the two groups were observed, and the blood glucose indicators in OGTT at 24 to 28 weeks of the two groups were evaluated. Results The fasting blood glucose in the early pregnancy in the control group was lower than that in the observation group(P<0.05), and the blood glucose values before the oral administration of glucose, in 1 h and 2 h after oral administration in the control group were lower than those in the observation group(P<0.05). Conclusion The fasting blood glucose screening in the early pregnancy for delivery women with gestational diabetes can increase the blood glucose, and the blood glucose indicators before administration of glucose and in 1 h and 2 h after administration obviously increase for 75 g OGTT at 24 to 28 pregnancy weeks.
