摘要
目的对不同年龄段女性患者行种植修复并进行远期随访,观察种植体颈部骨吸收情况,并进一步分析其与雌激素水平变化的关系。方法抽取女性患者108人,植入种植牙共124颗。患者根据年龄分为三组:20~30岁为A组,共33人,植入39颗种植体,40-50岁为B组,共37人,植入42颗种植体,60岁以上为C组,共38人,植入43颗种植体。所有患者常规植入Axiom REG种植体(Anthogyr,法国),安装覆盖螺丝,术后CBCT检查确认种植体植入位置理想。上端修复选用氧化锆全冠(VITA,德国)。术后1年、5年进行随访,行CBCT检查测量比较不同组间种植体颈部骨吸收情况,同时检测患者外周血雌激素水平,分析种植体颈部骨吸收与雌激素水平的关系。结果 1、1年随访时,三组种植体均存留。5年随访时,A组和B组种植体无脱落,C组种植体脱落1颗,三组成功率差别无统计学意义(P>0.05)。2、颈部骨吸收情况:1年随访时,颈部平均骨吸收高度、雌激素水平分别为:A组为0.23±0.12mm、53.5±2.1ng/L,B组为0.41±0.14mm、50.4±1.6ng/L,C组为0.47±0.12mm、37.2±2.1ng/L。5年随访时,颈部平均骨吸收高度、雌激素水平分别为:A组为0.28±0.56mm、52.1±1.8ng/L,B组为0.84±0.13mm、44.3±0.8ng/L,C组为1.05±0.12mm、30.6±1.2ng/L。三组间差异具有统计学意义(P<0.05),并且骨吸收程度与E2水平走向相反。结论女性患者种植体颈部骨吸收情况与雌激素水平密切相关。
