自由体位分娩法联合分娩减痛法对阴道分娩产妇分娩结局的影响

摘要
目的探讨自由体位分娩法联合分娩减痛法对阴道分娩产妇分娩结局的影响。方法将我院妇产科收治的116例拟行阴道分娩产妇按随机数表法分为对照组(58例,常规助产)与观察组(58例,自由体位联合分娩减痛法)。比较两组产妇的分娩情况。结果观察组产妇阴道自然分娩率、阴道分娩会阴Ⅰ度裂伤率显著高于对照组,剖宫产率、会阴侧切率低于对照组,产后2 h出血量少于对照组(P<0.05);观察组产妇第一、二、三产程及总产程时间均短于对照组(P<0.05);干预后,两组产妇PPI、VAS评分及PRI感觉、PRI情感总分及PRI总分均显著降低,且观察组低于对照组(P<0.05)。结论自由体位分娩法联合分娩减痛法能缓解阴道分娩产妇分娩疼痛,缩短产程,改善妊娠结局。
Objective To investigate the effect of free posture delivery combined with delivery analgesia method on delivery outcome of vaginal delivery puerpera. Methods A total of 116 puerpera undergoing vaginal delivery admitted in the obstetrics and gynecology department of our hospital were divided into control group(58 cases, routine midwifery) and observation group(58 cases, free posture delivery combined with delivery analgesia method) according to the random number table method. The delivery conditions of the two groups were compared. Results The natural vaginal delivery rate and the perineal laceration rate of Ⅰdegree in the observation group were significantly higher than those in the control group, the rate of cesarean section and episiotomy in the observation group were significantly lower than those in the control group, and the haemorrhagia amount of two hours after delivery in the observation group was less than those in the control group(P<0.05). The duration of the 1, 2, 3 stages of labor and the total stage of labor in the observation group were shorter than those in the control group(P<0.05). After intervention, the scores of PPI, VAS, the total score of PRI sensation,PRI emotion and PRI in the two groups decreased significantly, and those in the observation group were lower than the control group(P<0.05). Conclusion Free posture delivery combined with delivery analgesia method can alleviate vaginal delivery pain, shorten the labor process and improve pregnancy outcomes of puerpera.
