血清DKK1、CA72-4、GAS-17的表达情况及与胃癌患者疾病特征的关系

摘要
目的探讨胃癌患者血清Dickkopf相关蛋白1(DKK1)、糖类抗原72-4(CA72-4)、促胃液素-17(GAS-17)的水平变化及与胃癌患者疾病特征的关系。方法采集90例胃癌患者(胃癌组)、90例胃溃疡患者(溃疡组)和90例健康体检者(对照组)的静脉血,检测血清DKK1、CA72-4、GAS-17水平,并分析其水平变化与胃癌患者TNM分期、分化程度和淋巴结转移的关系。结果胃癌组患者血清DKK1、CA72-4、GAS-17水平均高于溃疡组患者和对照组受试者,且胃溃疡组患者CA72-4、GAS-17水平均高于对照组受试者,差异均有统计学意义(P﹤0.05)。TNM分期为Ⅰ~Ⅱ期、分化程度为中高分化胃癌患者的血清DKK1、CA72-4、GAS-17水平均明显低于TNM分期为Ⅲ~Ⅳ期的患者、分化程度为低分化的患者,无淋巴结转移胃癌患者血清DKK1、CA72-4水平均明显低于有淋巴结转移的患者(P﹤0.01);但有淋巴结转移和无淋巴结转移胃癌患者血清GAS-17水平比较,差异无统计学意义(P﹥0.05)。结论胃癌患者血清DKK1、CA72-4、GAS-17水平较高,且可能与胃癌的临床分期、分化程度和淋巴结转移情况有关。
        Objective To investigate the changes of serum Dickkopf-related protein 1(DKK1), carbohydrate antigen72-4(CA72-4) and gastrin-17(GAS-17) in patients with gastric cancer and their relationship with disease characteristics.Method Ninety patients with gastric cancer(gastric cancer group), 90 patients with gastric ulcer(ulcer group), and 90 healthy subjects(control group) were included in the study and had their intravenous blood collected to test the serum levels of DKK1, CA72-4, and GAS-17, furthermore, the relationship between the changes of serum DKK1, CA72-4, and GAS-17 in gastric cancer patients and TNM stages, degree of differentiation, and lymph node metastasis was investigated. Result The levels of serum DKK1, CA72-4 and GAS-17 in the gastric cancer group were significantly higher than those in the ulcer group and the control group, besides, the levels of CA72-4 and GAS-17 in ulcer group were significantly higher than those in control group, with statistically significant differences observed(P<0.05). The levels of serum DKK1, CA72-4, and GAS-17 in patients with gastric cancer with TNM stage I-II, and moderate to high degree of differentiation were significantly lower compared to patients with TNM stage III-IV, and poor differentiation disease, negative lymph node metastasis in gastric cancer patients was associated with significantly lower levels of serum DKK1 and CA72-4 compared to positive lymph node metastasis(P<0.01); while the GAS-17 level was independent of the lymph node metastasis among these patients(P>0.05). Conclusion The levels of serum DKK1, CA72-4 and GAS-17 in gastric cancer patients are significantly increased, and may be closely related to the clinical stage, degree of differentiation and lymph node metastasis.
