章勤辨证治疗经期延长经验

摘要
[目的]浅析浙江省名中医章勤教授临床辨证施治经期延长的经验。[方法]分析章师对经期延长的病因病机认识,阐述在临床实践中的治疗方法和治疗原则,并通过举隅章师临床治疗经期延长的医案,结合跟随临诊的体会,分析总结章师治疗经期延长的临床经验。[结果]章师认为经期延长的病因病机主要是冲任不固,经血失约,其发病与脾肾气虚、瘀血阻滞、阴虚内热有关,提出临床治疗应以调和气血为本,兼以补益脏腑,收敛固摄。所举案例一为气虚血瘀型,治以益气活血,祛瘀止血;一为气阴两虚型,治以益气滋阴,收敛止血。经治疗后患者经期恢复正常。[结论]章师治疗经期延长坚持辨证审因,热者清之,寒者温之,虚者补之,瘀者消之,治疗得宜;同时病证结合,中医辨证与西医病症参合,拓展临床辨证思路。遣方精妙,用药灵活,兹获良效,进一步阐明了中医对经期延长的辨证论治和用药特点,值得深入学习和推广应用。
[Objective]To summarize the experience of Professor ZHANG Qin,a famous Chinese doctor in Zhejiang Province,in the treatment of menostaxis by clinical syndrome differentiation.[Method]The article analyzes the cognition of etiology and pathogenesis of menostaxis,as well as clinical therapeutic methods and principles.Through the cases of teacher ZHANG treating menostaxis,combined with the usual clinical experience,we analyze and summarize teacher ZHANG's clinical experience of the treatment of menostaxis.[Results]Professor ZHANG considers that menostaxis is basically caused by insufficiency of Chong channel and Ren channel failing to restrain menstruation.Its pathological occurrence is related to Qi deficiency of spleen and kidney,blood stasis block and internal heat due to Yin deficiency.It's put forward that clinical treatment should take regulation of Qi and blood as the foremost,as well as tonifying viscera and bowels and astringing for hemosatasis.Two clinical cases are given in the article,one of which is caused by deficiency of Qi and blood stasis,treated by tonifying Qi and promoting blood circulation to remove blood stasis and stop bleeding.And the other is caused by deficiency of Qi and Yin,treated by nourishing Qi and Yin to astringe and stanch.Patients in the cases all achieved satisfactory effect.[Conclusion]Professor ZHANG insists that treating menostaxis should be based on syndrome differentiation with adaptive therapy,such as treating heat with cold drugs,treating cold with hot drugs,treating deficiency with tonifying drugs and treating blood stasis with drugs which invigorate the circulation of blood.Meanwhile,Professor ZHANG also emphasizes that syndrome and disease should be clinically combined to explore better treatments.The experience of Professor ZHANG has distinct features and positive effects,with prescriptions and medicine taken ingeniously and flexibly,illuminating treatment based on syndrome differentiation and characteristic of using herb in TCM,so it 's worth studying deeply and applying profoundly.
